Özet

Hepatobiliyer sistem hastalıklarının tanı ve takibinde nükleer tıp yöntemleri, fonksiyonel ve moleküler düzeyde kritik bilgiler sunmaktadır. Hepatobiliyer sintigrafi, özellikle akut/kronik kolesistit, biliyer atrezi ve safra kaçaklarının değerlendirilmesinde yüksek duyarlılığa sahip invaziv olmayan bir yöntemdir. Karaciğer fonksiyonlarını ve safra akış dinamiklerini gerçek zamanlı izleyerek anatomik detayların ötesinde fizyolojik veriler sağlar. FDG-PET/BT ve PET/MR ise hücresel glukoz metabolizmasını aydınlatarak primer karaciğer tümörleri, kolanjiokarsinomlar, safra kesesi kanserleri ve metastatik odakların saptanmasında önemli rol oynar. Bu ileri görüntüleme yöntemleri, lezyonların malign-benign ayrımının yapılmasına, doğru evrelemeye, tedaviye yanıtın izlenmesine ve prognoz tahminine olanak tanır. Tümör diferansiyasyonu ve intratümöral değişikliklere bağlı olarak FDG tutulum derecesi değişkenlik gösterse de, moleküler arka planın anlaşılması klinik stratejilerin belirlenmesinde ve cerrahi planlamada optimize edilmiş sonuçlar sağlar.

Nuclear medicine modalities offer critical functional and molecular insights for diagnosing and monitoring hepatobiliary system disorders. Hepatobiliary scintigraphy is a highly sensitive, non-invasive technique primarily utilized to evaluate acute/chronic cholecystitis, biliary atresia, and bile leaks. By monitoring liver function and bile flow dynamics in real time, it delivers vital physiological data beyond structural details. Concurrently, FDG-PET/CT and PET/MR imaging illuminate cellular glucose metabolism, playing a pivotal role in detecting primary liver malignancies, cholangiocarcinomas, gallbladder cancers, and metastatic lesions. These advanced diagnostic tools facilitate the differentiation between benign and malignant lesions, precise staging, treatment response evaluation, and prognostic assessments. Although FDG uptake intensity varies based on tumor differentiation and intratumoral alterations, understanding the underlying molecular mechanism significantly optimizes clinical strategy selection and pre-operative management.
