Özet

Karaciğerin paraziter hastalıkları, özellikle sosyoekonomik düzeyi düşük ve hijyen koşullarının yetersiz olduğu endemik bölgelerde önemli bir halk sağlığı sorunudur. Gastrointestinal sistem yoluyla bulaşan bu parazitler portal venöz sistem aracılığıyla karaciğere ulaşarak ciddi morbidite ve mortaliteye yol açmaktadır. Ülkemizde en sık kistik ekinokokoz görülmekle birlikte amip apseleri, fascioliasis ve ascariasis vakalarıyla da karşılaşılmaktadır. Cestodlar grubundan Echinococcus granulosus kistik, E. multilocularis ise maligniteyi taklit eden alveolar ekinokokoza neden olur; tedavilerinde albendazol gibi medikal ajanlar, perkutan girişimler (PAIR) ve cerrahi yöntemler uygulanır. Trematodlardan Fasciola hepatica safra yollarını tutarak triklabendazol ile tedavi edilirken, Clonorchis sinensis ve Opisthorchis viverrini kronik süreçte kolanjiokarsinom riskini belirgin şekilde artırır. Schistosoma türleri ise periportal fibrozis ve portal hipertansiyona yol açabilmektedir. Nematodlardan Ascaris lumbricoides biliyer obstrüksiyona, protozoalardan Entamoeba histolytica ise özellikle erkeklerde daha sık olmak üzere sağ lobda amip apselerine neden olur. Erken tanı, doğru skolisidal solüsyon kullanımı ve uygun tedavi komplikasyonları önlemede kritiktir.

Parasitic diseases of the liver represent a significant public health issue, particularly in endemic regions with low socioeconomic status and inadequate hygiene conditions. Transmitted through the gastrointestinal tract, these parasites reach the liver via the portal venous system, causing substantial morbidity and mortality. While cystic echinococcosis is the most common parasitic liver disease in Turkey, amoebic abscesses, fascioliasis, and ascariasis are also encountered. Among cestodes, Echinococcus granulosus causes cystic echinococcosis, whereas E. multilocularis leads to alveolar echinococcosis mimicking malignancy; their management includes medical agents like albendazole, percutaneous interventions (PAIR), and surgery. Regarding trematodes, Fasciola hepatica affects the biliary system and responds to triclabendazole, while Clonorchis sinensis and Opisthorchis viverrini significantly increase the risk of cholangiocarcinoma in chronic stages. Schistosoma species can induce periportal fibrosis and portal hypertension. Among nematodes, Ascaris lumbricoides causes biliary obstruction, and the protozoan Entamoeba histolytica triggers amoebic liver abscesses, predominantly in the right lobe and more frequently in males. Early diagnosis, precise utilization of scolicidal solutions, and tailored treatment are essential to mitigate complications.
