Karaciğer Kistlerinin Değerlendirilmesi
Özet
Karaciğerin kistik lezyonları, etiyoloji, klinik görünüm ve tedavi yaklaşımları açısından oldukça heterojen ve çoğunlukla rastlantısal olarak saptanan selim seyirli bir hastalık grubudur. En sık görülen benign oluşum, genellikle asemptomatik olan ve ultrasonografi ile maliyet-etkin şekilde teşhis edilebilen basit kistlerdir. Ancak, malign transformasyon riski taşıyan biliyer kistadenom ve kistadenokarsinom gibi neoplastik yapıların yanı sıra, paraziter kökenli kistik ve alveoler ekinokokkus olgularının doğru ayırt edilmesi kritik önem taşır. Ayrıca, intrahepatik safra kanallarının segmental kistik dilatasyonu ile karakterize Caroli hastalığı ve genetik geçişli polikistik karaciğer hastalığı da bu spektrumda yer alır. Tedavi yaklaşımları lezyonun türüne ve semptomlara göre değişiklik göstermekte; basit kistlerde bekle-gör politikası, skleroterapi veya laparoskopik fenestrasyon tercih edilirken, neoplastik ve şüpheli lezyonlarda altın standart cerrahi rezeksiyondur.
Cystic lesions of the liver represent a highly heterogeneous group of diseases in terms of etiology, clinical manifestation, and treatment, which are mostly diagnosed incidentally and follow a benign course. Simple cysts are the most common benign lesions; they are generally asymptomatic and accurately diagnosed using cost-effective ultrasonography. However, distinguishing simple cysts from neoplastic lesions with malignant potential, such as biliary cystadenoma and cystadenocarcinoma, as well as parasitic infections like cystic and alveolar echinococcosis, is of paramount importance. This clinical spectrum also encompasses Caroli’s disease, characterized by segmental cystic dilatation of intrahepatic bile ducts, and genetically transmitted polycystic liver disease. Management strategies vary significantly based on the specific type of lesion and symptoms, ranging from observation, sclerotherapy, or laparoscopic fenestration for simple cysts, to radical surgical resection as the gold standard for neoplastic and suspicious masses.
