Safra Kesesinin Anatomik Yapısı
Özet
Dünya genelinde en yaygın hepatobiliyer cerrahi operasyonu olan kolesistektomi ameliyatları açısından safra kesesi ve ekstrahepatik safra yollarının anatomik yapısının tam olarak bilinmesi büyük bir önem taşımaktadır. Safra kesesi, karaciğerin sağ lobunun altında yer alan; fundus, korpus, infundibulum ve kollum olmak üzere dört anatomik bölümden oluşan armut biçimli bir organdır. Organın en sık görülen konjenital varyasyonu fundus kısmındaki kıvrılma ile oluşan Frigya şapkası deformitesidir. Safra kesesi anatomisinde %65 oranında varyasyon görülebilmekte olup, cerrahide yaralanma riskini azaltmak adına Calot üçgeni ve kritik güvenli görüş kuralları büyük önem taşımaktadır. Arteriyel beslenmesini çoğunlukla sağ hepatik arterden köken alan sistik arter vasıtasıyla sağlayan safra kesesinin venöz drenajı doğrudan karaciğer parankimine olur. Ekstrahepatik safra yolları ise sağ ve sol hepatik kanallar, ortak hepatik kanal, sistik kanal ve koledok kanalından meydana gelmektedir. Safra kesesini ortak hepatik kanala bağlayan ve Heister plikalarını barındıran sistik kanal, anatomik olarak anguler, paralel veya spiral şekillerde birleşim gösterebilir. Supraduodenal, retroduodenal, pankreatik ve intramural bölümlerden oluşan koledok kanalı ise duodenumun ikinci kısmına açılarak safra akışını kontrol eder. Bu anatomik yapıların ve varyasyonların net olarak kavranması, cerrahideki başarıyı artırmak ve postoperatif safra kaçağı gibi komplikasyonları önlemek için kritiktir.
Comprehensive knowledge of the anatomical structure of the gallbladder and extrahepatic biliary tract carries paramount importance for cholecystectomy, the most common hepatobiliary surgical procedure worldwide. The gallbladder is a pear-shaped organ located beneath the right lobe of the liver and is anatomically divided into four sections: fundus, corpus, infundibulum, and neck. The most frequent congenital variation of the organ is the Phrygian cap deformity, which occurs due to a fold in the fundus. Variations are observed in approximately 65% of gallbladder anatomies; therefore, understanding the Calot's triangle and adhering to the critical view of safety guidelines are vital to minimize intraoperative injuries. The gallbladder is primarily supplied by the cystic artery, which usually originates from the right hepatic artery, while its venous drainage flows directly into the liver parenchyma. The extrahepatic biliary system consists of the right and left hepatic ducts, the common hepatic duct, the cystic duct, and the common bile duct. The cystic duct, containing the spiral valves of Heister, connects the gallbladder to the common hepatic duct and can join it in angular, parallel, or spiral patterns. The common bile duct comprises supraduodenal, retroduodenal, pancreatic, and intramural segments, opening into the second part of the duodenum to regulate bile flow. A thorough understanding of these anatomical variations is crucial to enhancing surgical success and preventing postoperative complications like bile leaks.
