Özet

Safra kesesi hastalıklarının tanısında kullanılan radyolojik görüntüleme yöntemleri, hastaların klinik yönetiminde ve doğru teşhiste kritik bir rol oynamaktadır. Kolelitiazis, akut/kronik kolesistit ve safra kesesi karsinomu gibi en sık karşılaşılan patolojilerde, düşük maliyeti ve iyonlaştırıcı radyasyon içermemesi nedeniyle ultrasonografi (US) ilk tercih edilen yöntemdir. US, safra taşları ve kolesistit tespitinde yüksek hassasiyete sahip olsa da, özellikle obez hastalarda ve kolesistit komplikasyonlarında bilgisayarlı tomografi (BT) daha etkin sonuçlar vermektedir. Yumuşak doku çözünürlüğü yüksek olan manyetik rezonans görüntüleme (MRG) ve MRCP ise safra yollarının anatomik ve fonksiyonel değerlendirmesinde US'den sonra en sık başvurulan yöntemler haline gelmiştir. Ayrıca frigya kepi, agenezi ve duplikasyon gibi konjenital anomaliler ile polipler ve adenomyomatozis gibi lezyonların ayrımında "inci kolye" veya "kuyruklu yıldız" gibi spesifik radyolojik işaretler yol göstericidir. ERCP ve PTK gibi invaziv yöntemler ise tanısal amaçtan ziyade tedavi ve stent uygulamalarında tercih edilir. Gangrenöz veya amfizematöz kolesistit gibi acil cerrahi gerektiren durumlarda doğru görüntüleme modalitesinin seçilmesi hayati önem taşımaktadır.

Radiological imaging modalities utilized in the diagnosis of gallbladder diseases play a critical role in clinical management and accurate evaluation. For the most common pathologies such as cholelithiasis, acute/chronic cholecystitis, and gallbladder carcinoma, ultrasonography (US) is the initial method of choice due to its low cost and lack of ionizing radiation. Although US has high sensitivity in detecting gallstones and cholecystitis, computed tomography (CT) provides more effective results in cases of obesity and cholecystitis complications. Magnetic resonance imaging (MRI) and MRCP, which offer high soft-tissue resolution, have become the most frequently used methods after US for the anatomical and functional assessment of the biliary system. Additionally, specific radiological signs like the "pearl necklace" or "comet tail" guide the differentiation of polyps, adenomyomatosis, and congenital anomalies such as Phrygian cap, agenesis, and duplication. Invasive methods like ERCP and PTK are preferred for therapeutic interventions and stenting rather than purely diagnostic purposes. Selecting the correct imaging modality is vital in conditions requiring emergency surgery, such as gangrenous or emphysematous cholecystitis.
