Safra Kesesi Karsinomunun Tanı ve Tedavisi
Özet
Safra kesesi karsinomu, biliyer sistem tümörleri arasında en sık karşılaşılan, yüksek morbidite ve mortalite oranlarına sahip malign bir tümördür. Hastalık genellikle geç semptom verdiğinden erken evrelerde asemptomatik seyretmekte ve çoğunlukla elektif kolesistektomiler sonrasında tesadüfen teşhis edilmektedir. Semptomlar ortaya çıktığında ise tümör genellikle ileri evreye ulaşmış olmaktadır. Etiyolojisinde en önemli risk faktörü kolelitiazis (safra taşları) olup, taşın boyutu ve bulunma süresi arttıkça kronik inflamasyona bağlı olarak kanser riski de artmaktadır. Bunun yanı sıra ileri yaş, kadın cinsiyeti, porselen safra kesesi, primer sklerozan kolanjit ve bazı bakteriyel enfeksiyonlar da gelişiminde rol oynayan diğer önemli faktörlerdir. Safra kesesi kanserinin tek küratif tedavi seçeneği cerrahi rezeksiyondur. Tümörün evresine göre basit kolesistektomiden, karaciğer yatağı rezeksiyonu ve lenf diseksiyonunu içeren radikal cerrahiye kadar farklı yöntemler uygulanır. Hastalık kemoterapi ve radyoterapiye karşı dirençli olduğundan, adjuvan tedavilerin sağ kalım üzerindeki etkinliği oldukça sınırlıdır. Bu nedenle, prognozu iyileştirmede en kritik basamak erken teşhistir.
Gallbladder carcinoma is the most common malignant tumor of the biliary tract, characterized by high morbidity and mortality rates. Due to its late presentation, the disease is generally asymptomatic in its early stages and is frequently diagnosed incidentally following elective cholecystectomy. By the time patients become symptomatic, the tumor has usually reached an advanced stage. Cholelithiasis (gallstones) represents the most significant etiological risk factor, where larger stone size and longer duration increase malignancy risk through chronic inflammation. Other notable risk factors include advanced age, female gender, porcelain gallbladder, primary sclerosing cholangitis, and specific bacterial infections. Surgical resection remains the only curative treatment modality for gallbladder cancer. Depending on the tumor stage, surgical options range from simple cholecystectomy to radical surgery involving liver bed resection and regional lymph node dissection. Because gallbladder tumors show limited responsiveness to chemotherapy and radiotherapy, the impact of adjuvant therapies on overall survival is restricted. Consequently, achieving an early diagnosis is the most critical factor in improving patient prognosis.
