Özet
Helikobakter pilori (H. pilori), dünya nüfusunun büyük bir kısmını etkileyen, mide ortamına yüksek uyum sağlamış spiral şekilli gram-negatif bir bakteridir. Erken yaşam dönemindeki sosyoekonomik yetersizlikler ve kalabalık yaşam koşulları bulaşma riskini artırırken; fekal-oral veya oral-oral yollarla kişiden kişiye yayılmaktadır. Bakteri, salgıladığı üreaz, katalaz ve fosfolipaz gibi enzimler ve VacA ile CagA gibi virülans faktörleri sayesinde mide mukozal bariyerini bozarak doku hasarına ve kronik inflamasyona yol açar. H. pilori enfeksiyonu çoğunlukla asemptomatik seyretse de tedavi edilmediğinde akut ve kronik gastrit, peptik ülser, demir eksikliği anemisi ve B12 vitamini eksikliği gibi sistemik sorunlara neden olabilir. Ayrıca, mide mukozasındaki sürekli bağışıklık uyarımı ve hücresel hasar; intestinal metaplazi, gastrik atrofi, mide lenfoması (MALToma) ve mide kanseri gelişimi ile güçlü bir şekilde ilişkilidir. Tanısında histoloji ve hızlı üreaz testi gibi invaziv yöntemlerin yanı sıra üre nefes testi ve dışkı antijen testi gibi non-invaziv aktif enfeksiyon testleri kullanılır. Tedavisi, antibiyotik direncine göre şekillenen proton pompa inhibitörleri ve çoklu antibiyotik kombinasyonlarından oluşan 14 günlük rejimleri içerir ve eradikasyon başarısı tedavi sonrası testlerle mutlaka doğrulanmalıdır.
Helicobacter pylori (H. pylori) is a highly adapted, spiral-shaped, gram-negative bacterium that infects a significant portion of the global population, causing chronic inflammatory disorders. Transmission primarily occurs from person to person through fecal-oral or oral-oral routes, with risk factors closely tied to low socioeconomic status and crowded living conditions during early childhood. The bacterium colonizes the gastric mucosa and induces tissue damage through key enzymes like urease, catalase, and phospholipase, alongside virulence factors such as VacA and CagA cytokines. While most infected individuals remain asymptomatic, untreated chronic inflammation can lead to acute or chronic gastritis, peptic ulcer disease, and extra-gastrointestinal manifestations including iron deficiency anemia and vitamin B12 malabsorption. Furthermore, persistent immune stimulation and DNA damage significantly elevate the risk of gastric atrophy, intestinal metaplasia, MALT lymphoma, and gastric adenocarcinoma. Diagnosis relies on invasive endoscopic methods like histology and rapid urease tests, or non-invasive active infection assays such as the urea breath test and stool antigen test. Eradication therapies involve 14-day multi-antimicrobial regimens combined with proton pump inhibitors, tailored to antibiotic resistance profiles, and treatment success must always be confirmed post-therapy.
