Özet

Jinekolojik kanserler, mortalite ve morbidite açısından dünyada ve ülkemizde kadın sağlığını tehdit eden önemli sağlık sorunları arasında ilk on sırada yer almaktadır. Sağlık sistemlerine büyük ekonomik ve fiziki yük getiren bu kanser türlerinin kontrol altına alınması amacıyla literatürde birincil, ikincil ve üçüncül olmak üzere üç korunma basamağı uygulanmaktadır. Birincil korunma; serviks, over, endometrium, vulva ve vajina gibi kanser türleri oluşmadan önce riskli davranışlardan uzak durmayı, sağlıklı yaşam tarzı geliştirmeyi ve özellikle HPV ilişkili jinekolojik kanserler için hayati önem taşıyan HPV aşılamasını içerir. İkincil korunma, asemptomatik dönemdeki kadınların erken teşhisini hedefler; serviks kanserinde Pap smear ve HPV-DNA testleri yaygın tarama yöntemi olarak kullanılırken, over ve endometrium gibi kanserlerde spesifik bir tarama testi olmaması nedeniyle genetik danışmanlık, riskli grupların takibi ve semptomların izlenmesi ön plana çıkar. Üçüncül korunma ise teşhis sonrası uygun tedavi yönetimini, cerrahi, kemoterapi, radyoterapi gibi multidisipliner yaklaşımları ve hastaların yaşam kalitesini artıracak palyatif bakım ile rehabilitasyon hizmetlerini kapsamaktadır. Sonuç olarak, kadın sağlığını derinden etkileyen bu hastalıkların yükünü azaltmak için jinekolojik kanser korunma hizmetlerinin her kademede sürekli, erişilebilir ve sürdürülebilir kılınması gerekmektedir.

Gynecological cancers rank among the top ten significant health problems threatening women's health worldwide and in Turkey in terms of mortality and morbidity. In order to control these types of cancers, which impose a major economic and physical burden on health systems, three levels of prevention—primary, secondary, and tertiary—are implemented in the literature. Primary prevention includes staying away from risky behaviors, developing a healthy lifestyle before cancers such as cervix, ovary, endometrium, vulva, and vagina develop, and HPV vaccination, which is vital for HPV-related gynecological cancers. Secondary prevention aims at the early diagnosis of asymptomatic women; while Pap smear and HPV-DNA tests are used as common screening methods in cervical cancer, due to the lack of a specific screening test in cancers such as ovarian and endometrial, genetic counseling, monitoring of risky groups, and symptom surveillance come to the fore. Tertiary prevention encompasses appropriate treatment management after diagnosis, multidisciplinary approaches such as surgery, chemotherapy, and radiotherapy, and palliative care and rehabilitation services that will improve the quality of life of patients. Consequently, to reduce the burden of these diseases deeply affecting women's health, gynecological cancer prevention services must be made continuous, accessible, and sustainable at every stage.
