Jinekolojik Kanserlerde Tarama ve Erken Tanı-2
Özet
Endometrium ve over kanserlerinde erken tanı ile tarama stratejilerini inceleyen bu çalışma, her iki jinekolojik kanser türünün yönetimini ele almaktadır. Gelişmiş ülkelerde sık görülen endometrium kanseri genellikle erken evrede anormal vajinal kanama ile belirti verirken, mortalitesi yüksek olan over kanseri sinsi seyri nedeniyle çoğunlukla ileri evrelerde teşhis edilmektedir. Endometrium kanserinin erken tanısında transvajinal ultrasonografi (TVS) ve Pipelle endometrial biyopsi gibi yöntemler kullanılmakta, genel popülasyonda rutin tarama önerilmezken Lynch sendromu gibi yüksek riskli gruplarda yıllık taramalar veya profilaktik cerrahi tavsiye edilmektedir. Over kanseri taramasında ise CA-125 tümör belirteci ve TVS kombinasyonları (ROCA algoritması gibi multimodal çalışmalar) üzerinde durulsa da, bu yöntemlerin genel popülasyonda mortaliteyi azaltmadığı, aksine gereksiz cerrahi müdahale ve komplikasyon riskini artırdığı gösterilmiştir. Bu nedenle uluslararası kılavuzlar (ACS, NICE, ESMO, US Preventive Services Task Force), semptomsuz ve ortalama risk altındaki kadınlarda her iki kanser türü için de rutin popülasyon taramasını önermemektedir. Sonuç olarak, her iki jinekolojik kanserde de epidemiyolojik, genetik ve moleküler faktörlere dayalı risk sınıflandırmalarının geliştirilmesi, gelecekteki tarama ve önleyici stratejilerin başarısı için kritik öneme sahiptir.
Focusing on early diagnosis and screening strategies in endometrial and ovarian cancers, this study addresses the management of both gynecological cancer types. While endometrial cancer, common in developed countries, usually presents at an early stage with abnormal vaginal bleeding, ovarian cancer, which has a high mortality rate, is mostly diagnosed at advanced stages due to its insidious course. Methods such as transvaginal ultrasonography (TVS) and Pipelle endometrial biopsy are used for the early diagnosis of endometrial cancer; while routine screening is not recommended for the general population, annual screenings or prophylactic surgery are advised for high-risk groups like Lynch syndrome. For ovarian cancer screening, although combinations of the CA-125 tumor marker and TVS (such as multimodal studies using the ROCA algorithm) have been emphasized, these methods have been shown not to reduce mortality in the general population, but rather to increase the risk of unnecessary surgical interventions and complications. Consequently, international guidelines (ACS, NICE, ESMO, US Preventive Services Task Force) do not recommend routine population screening for either type of cancer in asymptomatic women at average risk. In conclusion, improving risk stratification based on epidemiological, genetic, and molecular factors is of critical importance for the success of future screening and preventive strategies in both gynecological cancers.
