Jinekolojik Kanserlerde Radyoterapi ve Hemşirelik Bakımı

Özet

Bu bölüm, jinekolojik kanserlerin tedavisinde sıkça başvurulan bir yöntem olan radyoterapi sürecinde hemşirelik bakımının önemini ve yönetimini ele almaktadır. Radyoterapi; eksternal, brakiterapi ve intraoperatif olmak üzere farklı tekniklerle uygulanmakta ve serviks, endometrium, over, vulva ile vajen kanserlerinde tedavi veya semptom hafifletme amacıyla endike olmaktadır. Tedavi süresince radyasyon hem kanser hücrelerini hem de normal dokuları etkileyerek hastaların yaşam kalitesini düşüren çeşitli akut ve kronik yan etkilere yol açmaktadır. Bu süreçte hemşireler; tedavi öncesinde hastayı bilgilendirip kaygısını azaltmak, tedavi sırasında ve sonrasında ise cilt reaksiyonları, beslenme sorunları, gastrointestinal sistem bozuklukları, yorgunluk, psikososyal problemler, kronik pelvik ağrı, cinsel ve alt üriner sistem sorunları, trombositopeni, nötropeni ve lenfödem gibi komplikasyonları yönetmekle sorumludur. Özellikle vajinal dilatör kullanımı, cilt hijyeninin sağlanması, beslenme düzenlemeleri ve enfeksiyon kontrolü gibi spesifik hemşirelik girişimleri semptom yönetiminde kritik bir role sahiptir. Sonuç olarak, multidisipliner bir yaklaşımla sunulan bireyselleştirilmiş hemşirelik bakımı, hastaların tedaviye uyumunu artırmada, komplikasyonları azaltmada ve başarıyı desteklemede temel bir unsurdur.

This chapter addresses the significance and management of nursing care during radiotherapy, a frequently utilized method in treating gynecological cancers. Radiotherapy is administered through various techniques, including external, brachytherapy, and intraoperative, and is indicated for treatment or symptom palliation in cervix, endometrium, ovarian, vulvar, and vaginal cancers. Throughout the process, radiation affects both cancer cells and normal tissues, leading to various acute and chronic side effects that diminish patients' quality of life. In this period, nurses are responsible for informing patients prior to treatment to alleviate anxiety, and managing complications during and after therapy, such as skin reactions, nutritional issues, gastrointestinal disorders, fatigue, psychosocial problems, chronic pelvic pain, sexual and lower urinary tract dysfunctions, thrombocytopenia, neutropenia, and lymphedema. Specific nursing interventions, particularly the use of vaginal dilators, maintaining skin hygiene, nutritional adjustments, and infection control, play a critical role in symptom management. In conclusion, individualized nursing care delivered through a multidisciplinary approach is a fundamental element in enhancing patients' treatment adherence, minimizing complications, and supporting therapeutic success.
