PERİNATAL DÖNEMDE PSİKOLOJİK DEĞİŞİMLER VE HEMŞİRELİK YAKLAŞIMLARI

Özet

Perinatal dönem; gebelik, doğum ve doğum sonrası ilk yılı kapsayan, kadın hayatında hem fizyolojik hem de psikososyal açıdan derin değişimlerin yaşandığı gelişimsel bir geçiş sürecidir. Bu hassas süreçte kadınların yaklaşık %15-20'sinde, uyum problemlerinden postpartum psikoza kadar uzanan çeşitli ruh sağlığı sorunları gelişebilmektedir. İlk trimesterde gebeliğin kabulü ve ambivalan duygular öne çıkarken, son trimesterde doğum korkusu ve anksiyete tetiklenebilmektedir. Doğum eyleminin kendisi bazı kadınlar tarafından travmatik bir deneyim olarak algılanabilmekte ve Travma Sonrası Stres Bozukluğu’na (TSSB) yol açabilmektedir. Doğum sonrasında ise hafif ve geçici bir düşük ruh hali olan postpartum hüzün sıklıkla görülürken, daha şiddetli tablolarda postpartum depresyon ve gerçekliği ayırt edememe ile karakterize postpartum psikoz gibi acil psikiyatrik durumlar ortaya çıkabilmektedir. Kadın sağlığı hemşireleri, perinatal iyilik halinin korunması ve sürdürülmesinde kritik bir role sahiptir. Hemşireler, antenatal eğitim sınıfları ve danışmanlık yoluyla risk faktörlerini erken dönemde tanılama, stresi ve anksiyeteyi azaltma, uyumu artırma ve normalden sapma durumlarında bireyleri doğru ruh sağlığı uzmanlarına yönlendirme görevlerini üstlenerek bütüncül bir bakım sunmaktadır.

The perinatal period, encompassing pregnancy, childbirth, and the first postpartum year, is a developmental transition in a woman's life marked by profound physiological and psychosocial changes. During this sensitive process, approximately 15-20% of women may develop various mental health problems ranging from adaptation issues to postpartum psychosis. While acceptance of pregnancy and ambivalent emotions are prominent in the first trimester, childbirth fear and anxiety can be triggered in the final trimester. The experience of childbirth itself can be perceived as traumatic by some women, potentially leading to Post-Traumatic Stress Disorder (PTSD). Following delivery, postpartum blues—a mild and transient low mood—is frequently observed, whereas more severe conditions like postpartum depression and postpartum psychosis, an emergency psychiatric state characterized by a loss of reality, can arise. Women's health nurses play a critical role in preserving and promoting perinatal well-being. Through antenatal education classes and counseling, nurses provide holistic care by identifying risk factors early, reducing stress and anxiety, enhancing adaptation, and guiding individuals to appropriate mental health professionals in cases of deviation from normal health statuses.
