İNFERTİLİTE
Özet
İnfertilite, 12 ay boyunca düzenli ve korunmasız cinsel ilişkiye rağmen klinik bir gebeliğin gerçekleşmemesi durumudur. Üreme çağındaki çiftlerin yaklaşık %8-12'sini etkileyen bu hastalık; kadın faktörüne, erkek faktörüne, her ikisinin kombinasyonuna bağlı olarak ortaya çıkabileceği gibi açıklanamayan nedenlerden de kaynaklanabilir. Kadınlarda doğurganlık 35 yaşından sonra kademeli olarak azalırken, erkeklerde sperm kalitesindeki düşüş genellikle 60 yaşından önce fertiliteyi ciddi düzeyde etkilemez. Teşhis süreci; erkeklerde spermiogram testi ile semenin incelenmesini, kadınlarda ise ovulasyon fonksiyonlarının, over rezervinin, tubal ve uterin faktörlerin çeşitli hormonal ve görüntüleme yöntemleriyle (HSG, USG vb.) değerlendirilmesini içerir. Tedavi yaklaşımları medikal tedaviler, cerrahi müdahaleler, İntrauterin İnseminasyon (IUI) ve İn Vitro Fertilizasyon (IVF) gibi Üremeye Yardımcı Tedavi Yöntemlerini (ÜYTE) kapsamaktadır. Bu süreç, çiftlerde anksiyete, depresyon ve cinsel stres gibi ciddi psikososyal sorunlara yol açabilir. Hemşireler, tanı işlemlerinden erken gebelik dönemine kadar çiftlere bütüncül bakım sunmada, ilaç yönetiminde ve sürekli duygusal destek sağlamada anahtar bir role sahiptir.
Infertility is a disease characterized by the failure to achieve a clinical pregnancy after 12 months of regular, unprotected sexual intercourse. Affecting approximately 8-12% of couples of reproductive age, it can arise from female factors, male factors, a combination of both, or remain unexplained. While female fertility gradually declines after the age of 35, the decline in male sperm quality generally does not affect fertility significantly before the age of 60. Diagnostic procedures involve evaluating the semen through a spermiogram for men, and assessing ovulatory functions, ovarian reserve, tubal, and uterine factors using various hormonal and imaging methods (HSG, USG, etc.) for women. Treatment approaches encompass medical therapies, surgical interventions, Intrauterine Insemination (IUI), and Assisted Reproductive Technologies (ART) such as In Vitro Fertilization (IVF). This process can cause severe psychosocial distress, including anxiety, depression, and sexual stress in couples. Nurses play a pivotal role in delivering holistic care from diagnostic testing to early pregnancy, managing medications, and providing continuous emotional support.
