Özet

Radikülopati ve pleksopati belirtileriyle başvuran hastalarda, ayrıntılı öykü ve fizik muayene ile anatomik lokalizasyon yapıldıktan sonra uygun görüntüleme yöntemi belirlenmelidir. Toplumda asemptomatik dejeneratif değişikliklerin sık görülmesi nedeniyle, radikülopati tanısı tek başına görüntüleme bulgularına dayandırılmamalı, klinik şüpheyi desteklemek amacıyla kullanılmalıdır. Manyetik rezonans görüntüleme (MRG); yumuşak doku vizüalizasyonundaki üstünlüğü, radyasyon maruziyeti yaratmayışı ve non-invazif oluşuyla omurganın dejeneratif hastalıkları, radikülopati ve pleksusların kapsamlı değerlendirilmesinde kilit role sahiptir. Disk herniasyonları, Combined Task Force sınıflamasına göre bulging, protrüde disk, ekstrüde disk ve sekestrasyon olarak tanımlanır. Kemik dokuyu göstermede üstün olan Bilgisayarlı Tomografi (BT) ve MRG'nin kontrendike olduğu veya metal implant bulunduğu durumlarda tercih edilen BT-miyelografi diğer önemli yöntemlerdir. Floroskopi dinamik değerlendirme ve sinir kökü blokajı imkanı sunarken; ultrasonografi (USG) ise özellikle son yıllarda brakial pleksus ve periferik sinir hastalıklarında invazif olmayan, ucuz ve dinamik bir alternatif olarak öne çıkmaktadır. Disk dejenerasyonuna eşlik eden vertebral end plato değişiklikleri ise Modic Tip I (ödem), Tip II (yağlı değişiklik) ve Tip III (skleroz) olarak üç paternde sınıflanır.

In patients presenting with symptoms of radiculopathy and plexopathy, the appropriate imaging method must be determined after establishing anatomical localization through a detailed history and physical examination. Due to the high prevalence of asymptomatic degenerative changes in the population, the diagnosis of radiculopathy should not rely solely on imaging findings but should be used to support clinical suspicion. Magnetic resonance imaging (MRI) plays a key role in the comprehensive evaluation of degenerative spine diseases, radiculopathy, and plexuses due to its superiority in soft tissue visualization, lack of radiation exposure, and non-invasive nature. Disc herniations are defined as bulging, protruded disc, extruded disc, and sequestration according to the Combined Task Force classification. Computed Tomography (CT), which is superior in demonstrating bone tissue, and CT-myelography, preferred when MRI is contraindicated or in the presence of metal implants, are other important methods. While fluoroscopy offers dynamic evaluation and selective nerve root blockade; ultrasonography (USG) has emerged, especially in recent years, as a non-invasive, inexpensive, and dynamic alternative for brachial plexus and peripheral nerve diseases. Vertebral endplate changes accompanying disc degeneration are classified into three patterns as Modic Type I (edema), Type II (fatty changes), and Type III (sclerosis).
