Özet

Torakal radikülopati, servikal ve lomber bölgelere kıyasla göğüs kafesinin sağladığı rijit yapı nedeniyle nadir görülen bir omurga hastalığıdır. En sık nedeni torasik disk hastalığı olmakla birlikte, diyabetik radikülopati, metastatik tümör basıları, omurga kırıkları ve herpes zoster gibi enfeksiyonlar da etiyolojide önemli rol oynar. Genellikle T8 seviyesinin altında gelişen bu patoloji; sırt, göğüs ve karın bölgesinde bant şeklinde yayılan dermatomal ağrı, uyuşukluk, parestezi ve kas güçsüzlüğü ile kendini gösterir. Atipik ağrı karakteri nedeniyle miyokard enfarktüsü, peptik ülser veya plörit gibi hayati hastalıklarla karıştırılabilmekte ve teşhisi gecikebilmektedir. Tanı sürecinde kapsamlı bir anamnez ve fizik muayenenin yanı sıra, ilk tercih olarak spinal MRG, kemik yapılar için bilgisayarlı tomografi ve elektrofizyolojik değerlendirme amacıyla paraspinal ile interkostal kasları içeren iğne EMG'si uygulanır. Ağır kord basısı durumlarında ise miyelopati bulguları, paraparezi ve sfinkter bozuklukları kliniğe eşlik edebilir.

Thoracic radiculopathy is a rare spinal disorder compared to cervical and lumbar regions due to the rigid structure provided by the rib cage. Although its most common cause is thoracic disc disease, diabetic radiculopathy, metastatic tumor compressions, vertebral fractures, and infections such as herpes zoster also play an important role in its etiology. Generally developing below the T8 level, this pathology manifests with dermatomal pain radiating in a band-like pattern across the back, chest, and abdomen, alongside numbness, paresthesia, and muscle weakness. Due to its atypical pain character, it can be confused with vital conditions such as myocardial infarction, peptic ulcer, or pleurisy, which often leads to delayed diagnosis. In the diagnosis process, in addition to a comprehensive history and physical examination, spinal MRI is utilized as the first choice, followed by computed tomography for bony structures, and needle EMG encompassing paraspinal and intercostal muscles for electrophysiological evaluation. In cases of severe cord compression, myelopathy findings, paraparesis, and sphincter disturbances may accompany the clinical presentation.
