Özet

Ortodontide dental darlıkların ve posterior çapraz kapanışların tedavisinde kullanılan dental genişletme yöntemleri, tarihsel gelişimi ve klinik yaklaşımları bu çalışmada incelenmektedir. Yüzün transversal yöndeki büyümesinin erken durmasıyla oluşan üst çene darlıkları, 18. yüzyıldan itibaren Pierre Fauchard'ın Bandeau apareyi gibi çeşitli hareketli ve sabit aygıtlarla tedavi edilmeye başlanmıştır. Günümüzde dental genişletme; Schwarz, Coffin, Frankel ve Crozat gibi hareketli apareylerin yanı sıra Quad-heliks, Bi-heliks, Hexa-heliks, Transpalatal ark (TPA), W ark, Porter, Spring Jet ile nikel-titanyum esaslı sabit aygıtlar vasıtasıyla gerçekleştirilmektedir. Hafif ve fizyolojik kuvvetlerin uygulandığı bu prosedürler, genellikle 2 ila 6 ay sürerek posterior dişlerin bukkale devrilmesi veya dikleştirilmesiyle dental ark uzunluğunu artırmaktadır. Erken süt ve karışık dişlenme döneminde (özellikle 9 yaş öncesi) uygulanan transversal genişletme, maksiller segmentlerde ortopedik ayrılma da sağlayarak kalıcı sonuçlar sunmakta, mandibular asimetri ve eklem rahatsızlıklarını önlemektedir. Erişkin hastaların şiddetli darlıklarında cerrahi destek gerekebilirken, erken müdahale normal oklüzyon gelişimi, çapraşıklıkların giderilmesi ve solunum fonksiyonlarının iyileştirilmesi açısından kritik önem taşımaktadır.

Dental expansion methods, their historical development, and clinical approaches used in orthodontics to treat dental narrowness and posterior crossbites are examined in this study. Maxillary narrowness, which occurs due to the early cessation of transverse facial growth, has been treated since the 18th century using various removable and fixed appliances, starting with Pierre Fauchard's Bandeau appliance. Today, dental expansion is performed through removable appliances such as Schwarz, Coffin, Frankel, and Crozat, as well as fixed devices including Quad-helix, Bi-helix, Hexa-helix, Transpalatal arch (TPA), W arch, Porter, Spring Jet, and nickel-titanium-based systems. These procedures, which apply light and physiological forces, generally last between 2 to 6 months and increase dental arch perimeter by tipping or uprighting posterior teeth buccally. Transverse expansion applied during the early primary and mixed dentition periods (especially before age 9) provides orthopedic separation in maxillary segments, yields more stable results, and prevents mandibular asymmetry and joint disorders. While surgical assistance may be required for severe narrowness in adult patients, early intervention is of critical importance for normal occlusion development, alleviating crowding, and improving respiratory functions.
