Özet

Yavaş üst çene genişletmesi (YÜÇG), posterior çapraz kapanış ve transversal maksiller darlıkların tedavisinde süt, karma ve daimi dentisyon dönemlerinde yaygın olarak kullanılan fizyolojik bir ortodontik yaklaşımdır. Bu yöntem, çevre dokularda düşük direnç oluşturarak yan etkileri en aza indirmeyi, hafif ve sürekli fizyolojik kuvvetlerle doku yeniden şekillenmesini sağlamayı amaçlar. Tedavide Quadheliks, Nikel-Titanyum palatal genişletme apareyleri (NPE), Minne, yaylı genişleticiler, transpalatal ark, Leaf Expander ve akrilik plaklar gibi sabit veya hareketli birçok farklı aparey kullanılmaktadır. YÜÇG prosedürünün en önemli avantajları arasında destek dişlerde minimum devrilme (tipping) ve stres oluşturması, ağrı hissinin az olması ve hızlı üst çene genişletmesine (HÜÇG) kıyasla daha yüksek fizyolojik stabilite sunarak relaps (geri dönme) potansiyelini önemli ölçüde azaltması yer alır. Erken dönem uygulamalarında maksiller süturların açılmasıyla iskeletsel etkiler de elde edilebilirken, ileri yaşlarda ve erişkin hastalarda kortikotomi veya mini vida desteğiyle ya da cerrahisiz olarak başarılı dentoalveoler genişletmeler yapılabilmektedir. Literatürde YÜÇG'nin uzun dönem takipleri, elde edilen dental ark genişliğinin yüksek oranda korunduğunu ve hastaların tedaviye uyumunun mükemmel olduğunu göstermektedir.

Slow maxillary expansion (SME) is a physiological orthodontic approach widely used across deciduous, mixed, and permanent dentition stages for treating posterior crossbites and transverse maxillary constriction. This method aims to minimize side effects by generating low tissue resistance in surrounding structures, facilitating tissue remodeling through light and continuous physiological forces. Various fixed or removable appliances, such as the Quad-helix, Nickel-Titanium palatal expander (NPE), Minne expander, spring-loaded expanders, transpalatal arch, Leaf Expander, and acrylic plates, are utilized in this procedure. The primary advantages of SME include inducing minimal stress and tipping on anchor teeth, causing less pain and discomfort, and offering higher physiological stability compared to rapid maxillary expansion (RME), which significantly reduces the potential for relapse. While skeletal modifications can be achieved through midpalatal suture opening in young patients, successful dentoalveolar expansion can also be performed in adults either non-surgically or with the support of miniscrews and corticotomies. Long-term follow-up studies in the literature demonstrate that the dental arch width gained through SME is highly preserved over time, and patient compliance with this treatment methodology remains excellent.
