Post-COVID-19 syndrome is a clinical condition characterized by persistent symptoms lasting longer than 12 weeks following acute infection, including chronic fatigue, dyspnea, neurocognitive impairments, and multi-organ involvement. Pathophysiologically, excessive immune activation, cytokine storms, endothelial dysfunction, and systemic inflammatory processes play central roles. Since current pharmacological treatments remain insufficient in preventing persistent symptoms, tailored and symptom-adjusted exercise approaches emerge as an effective non-pharmacological treatment option. Regular and moderate exercise strengthens the immune system by stimulating the release of anti-inflammatory cytokines and delaying immunosenescence. Additionally, strength and endurance training reverse muscle atrophy and physical limitations while enhancing mitochondrial function and ventilatory capacity in the cardiovascular and pulmonary systems. Neurologically, exercise regulates brain structure and neurocognitive functions, alleviating anxiety, depression, and mental fatigue. Furthermore, advancements in wearable technologies and interdisciplinary training methods facilitate the safe implementation of personalized, home-based exercise programs. Consequently, supervised and individualized exercise programs represent a holistic and promising multi-systemic rehabilitation strategy to reduce the adverse effects of post-COVID-19 syndrome and improve patients' quality of life, functionality, and independence.
