Özet

Sinovyal sıvı analizi, eklem hastalıklarının, özellikle septik artrit ve kristal artropatilerinin (gut ve psödogut) ayırıcı tanısında kritik öneme sahip dinamik bir laboratuvar yöntemidir. Sağlıklı eklemlerde sürtünmeyi azaltan ve kıkırdağı besleyen bu sıvı, patolojik durumlarda lökosit sayısı, görünüm ve biyokimyasal özelliklerine göre inflamatuvar olmayan, inflamatuvar, septik ve hemorajik olmak üzere dört ana grupta sınıflandırılır. Makroskopik incelemede sıvının rengi, berraklığı ve viskozitesi değerlendirilirken; mikroskopik analizlerde monosodyum ürat (MSU) ve kalsiyum pirofosfat dihidrat (CPPD) kristallerinin tespiti ile lökosit sayımı gerçekleştirilir. Doğru tanı için örneklerin steril koşullarda, uygun tüplere (EDTA, heparin) alınarak hücre yıkımı ve kristal çözünmesini önlemek amacıyla süratle laboratuvara ulaştırılması gerekir. Ayrıca enfeksiyon şüphesinde Gram boyama, kültür ve PCR gibi mikrobiyolojik testler ile serum düzeyiyle karşılaştırmalı glukoz tayini tanısal doğruluğu optimize etmektedir.

Synovial fluid analysis is a critical diagnostic technique utilized to evaluate joint effusions and differentiate between conditions such as septic arthritis, crystal arthropathies (gout and pseudogout), and inflammatory or non-inflammatory arthritides. Physiologically acting as a plasma transudate that lubricates joints and nourishes cartilage, synovial fluid is categorized into non-inflammatory, inflammatory, septic, and hemorrhagic types based on visual and cellular traits. Macroscopic examination assesses clarity, color, and viscosity, while microscopic analysis primarily identifies monosodium urate (MSU) or calcium pyrophosphate dihydrate (CPPD) crystals and provides total leukocyte counts. To ensure diagnostic accuracy, samples must be collected under sterile conditions using appropriate tubes (EDTA or heparin) and promptly transported to prevent cellular degradation and crystal dissolution. Furthermore, microbiological investigations including Gram staining, cultures, and PCR, alongside biochemical evaluations like glucose comparisons with serum levels, remain indispensable for identifying underlying joint pathologies.
