Injuries to the four primary knee ligaments stabilizing the joint—the Anterior Cruciate Ligament (ACL), Medial Collateral Ligament (MCL), Posterior Cruciate Ligament (PCL), and Lateral Collateral Ligament (LCL)—are comprehensively analyzed regarding their anatomy, clinical examinations, diagnostics, and treatments. ACL tears usually stem from non-contact pivoting or sudden deceleration, diagnosed via Lachman and pivot shift tests. The MCL is the most frequently injured ligament but possesses a high healing capacity, often managed conservatively in low-grade sprains. The PCL, the strongest intra-articular ligament, is typically damaged through posterior tibial forces and assessed via the posterior sag test. LCL injuries are rare and frequently accompanied by peroneal nerve damage. While low-grade injuries are treated non-operatively with bracing and physical therapy, complete ruptures, persistent instability, or multi-ligament injuries generally require surgical repair or reconstruction.
