Obezite İçin Güncel Bilgiler
Özet
Obezite, dünya genelinde prevalansı giderek artan ve beraberinde Tip 2 diyabet, hipertansiyon ile kardiyovasküler hastalıklar gibi yandaş sorunlara yol açan kronik bir halk sağlığı problemidir. Tanısında Beden Kitle İndeksi (BKİ) standart ölçüm olarak kullanılırken, abdominal obezitenin belirlenmesinde bel çevresi ölçümü bağımsız bir risk göstergesidir. Obezitenin temel nedenleri enerji dengesizliği ve fiziksel aktivite yetersizliği olup, modern yaşamdaki beslenme değişiklikleri ile teknolojik araçlar bu süreci hızlandırmaktadır. Ayrıca bireylerde obeziteye genetik katkının yüzde 40 ila 70 arasında olduğu bilinmektedir. Türkiye’de yapılan TURDEP-I ve TURDEP-II çalışmaları, obezite sıklığının özellikle erkeklerde yüzde 107 oranında ciddi bir artış gösterdiğini ortaya koymuştur. Obezite, artmış yağ dokusu kitlesi ve patojen ürünlerin salınımı nedeniyle insülin direnci ve düşük dereceli inflamasyon gibi metabolik bozukluklara yol açarak beklenen yaşam süresini kısaltır. Hastalığın tedavisinde öncelikle diyet, egzersiz ve davranış değişikliğini içeren kapsamlı yaşam tarzı müdahaleleri uygulanmalıdır. Kilo kaybı, diyabetin remisyonunu sağlar ve kardiyovasküler risk faktörlerini azaltır. Yaşam tarzı değişikliklerine ek olarak Orlistat, Metformin ve GLP-1 reseptör agonistleri gibi ilaç tedavilerinden ya da BKİ değeri yüksek uygun hastalarda bariatrik cerrahi yöntemlerinden destek alınabilmektedir.
Obesity is a chronic public health problem with an increasing global prevalence, leading to comorbidities such as type 2 diabetes, hypertension, and cardiovascular diseases. While Body Mass Index (BMI) is used as a standard measure for diagnosis, waist circumference measurement serves as an independent risk indicator for evaluating abdominal obesity. The primary causes of obesity are energy imbalance and insufficient physical activity, with dietary changes in modern life and technological tools accelerating this process. Additionally, the genetic contribution to obesity in individuals is known to range between 40 and 70 percent. The TURDEP-I and TURDEP-II studies conducted in Turkey revealed that the prevalence of obesity has increased significantly, particularly by 107 percent in men. Obesity shortens life expectancy by causing metabolic disorders such as insulin resistance and low-grade inflammation due to increased adipose tissue mass and the release of pathogenic products. In treatment, comprehensive lifestyle interventions including diet, exercise, and behavior modification must be applied first. Weight loss provides diabetes remission and reduces cardiovascular risk factors. In addition to lifestyle changes, medical treatments like Orlistat, Metformin, and GLP-1 receptor agonists, or bariatric surgery methods for eligible patients with high BMI can be utilized.
