Medical toxicology is a critical area in emergency departments (ED) due to the challenges of managing poisoning, drug adverse effects, and overdoses. Accurate diagnosis is often hindered by unreliable patient histories, similar symptoms to other diseases, and the lack of in-hospital toxicology labs. Consequently, ED management requires a systematic approach beginning with immediate stabilization of airways, breathing, and circulation, followed by physical examinations and history taking from family or emergency staff. Decontamination techniques, such as gross, ocular, and gastrointestinal methods—including activated charcoal and whole-bowel irrigation—are utilized depending on the substance and patient status. For severe cases, enhanced elimination or Extracorporeal Membrane Oxygenation (ECMO) may provide critical respiratory and circulatory support. The chapter details specific treatment protocols for common toxic substances, including acetaminophen, cyclic antidepressants, salicylates, opioids, sympathomimetics, digitalis glycosides, and beta-blockers, highlighting antidote administration like N-acetylcysteine or naloxone as definitive interventions alongside supportive care. Ultimately, while supportive care and decontamination are sufficient for most intoxicated patients, specific antidotes remain the cornerstone of effective emergency toxicological treatment.
