Topikal D Vitamini Tedavisi, Tedavi Yönetimi ve Kullanıldığı Hastalıklar
Özet
Deride D vitamini analogları, keratinositler ve bağışıklık hücreleri üzerindeki D vitamini reseptörleri (VDR) ile etkileşime girerek proliferasyonu inhibe eder, farklılaşmayı uyarır ve antienflamatuvar etkiler gösterir. Plak tipi psöriyazis ve çeşitli dermatolojik hastalıkların tedavisinde kullanılan kalsitriol, kalsipotriol, maksakalsitol ve takalsitol gibi topikal ajanlar mevcuttur. Kalsitriol, kalsiyum homeostazı için gerekli aktif form olup kalsipotriole göre daha iyi bir güvenlik ve lokal tolerans profiline sahiptir. Sentetik bir türev olan kalsipotriol, psöriyaziste çok güvenli ve etkilidir; yüz ve hassas bölgeler için merhem formuna kıyasla daha az iritan olan ve kozmetik olarak tercih edilen krem formu çocuklarda ve kadınlarda öne çıkar. Maksakalsitol, keratinosit proliferasyonunu güçlü şekilde baskılarken kalsipotriole göre eritem ve skuamı azaltmada daha etkilidir. Takalsitol ise günlük tek doz rejimiyle hasta uyumu sağlar ve diğer analoglara kıyasla daha az deri iritasyonuna neden olur. Bu topikal vitamin D analogları, psöriyazis dışında liken planus, vitiligo, iktiyozlar, siğiller ve morfea gibi çok geniş bir dermatolojik hastalık grubunun tedavisinde de başarıyla uygulanmaktadır.
Vitamin D analogs in the skin exhibit anti-inflammatory effects, inhibit proliferation, and stimulate differentiation by interacting with vitamin D receptors (VDR) on keratinocytes and immune cells. Topical agents such as calcitriol, calcipotriol, maxacalcitol, and tacalcitol are utilized in the treatment of plaque psoriasis and various dermatological disorders. Calcitriol, the active form required for calcium homeostasis, offers similar efficacy but a significantly better safety and local tolerance profile compared to calcipotriol. Calcipotriol, a synthetic derivative, is highly effective in psoriasis; its cream formulation is less irritating than the ointment, preferred cosmetically, and suitable for sensitive areas, facial use, children, and female patients. Maxacalcitol strongly suppresses keratinocyte proliferation and demonstrates superior efficacy in reducing erythema and scaling compared to calcipotriol. Tacalcitol provides a compliance advantage with its once-daily regimen and causes less skin irritation than other analogs. Beyond psoriasis, these topical vitamin D analogs are successfully applied across a wide spectrum of dermatological conditions, including lichen planus, vitiligo, ichthyoses, warts, and morphea.
