Özet
D vitamini, kalsiyum ve fosfor metabolizmasında majör rol oynayan, iskelet sistemi dışında kardiyovasküler, otoimmün ve dermatolojik hastalıklarla da ilişkili yağda çözünen bir prohormondur. Temel görevi bağırsak, kemik ve böbrekten kalsiyum emilimini sağlayarak kemik sağlığını korumaktır. Hücre çekirdeklerinde D vitamini reseptörü içeren keratinositler sayesinde deride parakrin ve otokrin etkiler gösterir; proliferasyonu inhibe eder, farklılaşmayı uyarır ve bariyer fonksiyonunu destekler. Psöriasis, atopik dermatit ve vitiligo gibi birçok deri hastalığının tedavisinde topikal veya sistemik olarak kullanılır. D vitamininin asıl kaynağı deri olup, sentez stratum granulosum tabakasında ultraviyole B (UVB) ışınlarının etkisiyle kolesterolden (7-dehidrokolesterolden) başlar. Karaciğer ve böbrekteki hidroksilasyon süreçlerinin yanı sıra, deri de CYP27A1, CYP27B1 ve CYP24A1 enzimleri aracılığıyla kendi aktif metaboliti olan kalsitriolü üretebilir veya fazlasını katabolize edebilir. Sentez mekanizması kandaki kalsiyum, fosfor ve parathormon seviyeleriyle negatif feedback ile kontrol edilir. Vitamin D is a fat-soluble prohormone that plays a major role in calcium and phosphorus metabolism, and is associated with cardiovascular, autoimmune, and dermatological diseases beyond the skeletal system. Its primary function is to maintain bone health by ensuring calcium absorption from the intestines, bones, and kidneys. Through keratinocytes containing vitamin D receptors in their nuclei, it exhibits paracrine and autocrine effects in the skin, inhibiting proliferation, stimulating differentiation, and supporting barrier function. It is used topically or systemically in the treatment of various skin diseases such as psoriasis, topical dermatitis, and vitiligo. The main source of vitamin D is the skin, and its synthesis begins from cholesterol (7-dehydrocholesterol) in the stratum granulosum layer under the influence of ultraviolet B (UVB) radiation. In addition to hydroxylation processes in the liver and kidneys, the skin can also produce its own active metabolite, calcitriol, or catabolize the excess through CYP27A1, CYP27B1, and CYP24A1 enzymes. The synthesis mechanism is controlled via negative feedback based on blood calcium, phosphorus, and parathyroid hormone levels. Although sun protection is not thought to affect overall vitamin D levels, clearer data are required regarding the effects of high sun protection factor (SPF) sunscreens.
