Özet

Ürtiker, toplumda sık görülen ve yaşam kalitesini olumsuz etkileyen bir deri hastalığıdır. Son yıllarda yapılan çalışmalar, immünregülatuar ve antiinflamatuar etkileri olan D vitamininin ürtiker patogenezindeki ve tedavisindeki önemini ortaya koymaktadır. Araştırmalarda, serum D vitamini seviyesinin 30 ng/ml'nin altında olması yetersizlik veya eksiklik olarak kabul edilirken, kronik spontan ürtiker (KSÜ) hastalarında bu düzeylerin kontrol gruplarına kıyasla anlamlı derecede düşük olduğu saptanmıştır. D vitamini eksikliği, Th1/Th2 dengesini bozarak mast hücre degranülasyonuna ve inflamasyona yol açmaktadır. Akut ürtikerde D vitamini ile ilişki erişkinlerde net gösterilememişken, çocuklarda akut ürtiker süresi ile ters orantı bildiren çalışmalar mevcuttur. KSÜ hastalarında ise düşük D vitamini seviyelerinin hastalık şiddetiyle doğrudan ilişkili olabileceği belirtilmektedir. Yapılan klinik araştırmalar, standart ürtiker tedavilerine ek olarak uygulanan D vitamini takviyesinin (haftalık veya günlük yüksek dozlar), Ürtiker Aktivite Skorunu (UAS) ve inflamatuar biyobelirteçleri (IL-6, CRP, TNF-alfa) belirgin şekilde azalttığını göstermiştir. Sonuç olarak, D vitamini takviyesi KSÜ semptomlarını hafifletmede ve hastaların yaşam kalitesini iyileştirmede etkili ve güvenli bir destekleyici yöntemdir.

Urticaria is a common skin disease that negatively impacts the quality of life, and recent studies highlight the significance of vitamin D, known for its immunoregulatory and anti-inflammatory effects, in its pathogenesis and treatment. Research indicates that serum vitamin D levels are significantly lower in patients with chronic spontaneous urticaria (CSU) compared to control groups, with deficiency defined as levels below 20 ng/ml. This deficiency disrupts the Th1/Th2 balance, leading to mast cell degranulation and intense inflammation, while its correlation with acute urticaria remains non-significant in adults but inversely proportional to disease duration in children. Clinical trials demonstrate that adding vitamin D supplementation to standard urticaria treatments substantially reduces Urticaria Activity Scores (UAS) and serum inflammatory biomarkers such as IL-6, CRP, and TNF-alpha. Ultimately, vitamin D supplementation serves as an effective and safe adjunctive method to alleviate symptoms and improve the overall quality of life in CSU patients.
