Hidradenitis Süpürativa ve D Vitamini
Özet
Hidradenitis Süpürativa (HS), apokrin bezlerin yoğun olduğu bölgelerde puberteden sonra ortaya çıkan, ağrılı nodül ve fistüllerle seyreden kronik inflamatuar bir deri hastalığıdır. Genetik ve çevresel faktörlerin rol oynadığı HS patogenezinde doğal bağışıklık sistemi ve deri bariyer fonksiyonlarında bozukluklar gözlenmektedir. Son yıllarda yapılan çalışmalar, HS hastalarında genel popülasyona kıyasla D vitamini eksikliğinin çok daha yüksek oranlarda görüldüğünü ve serum D vitamini düzeyleri ile hastalık şiddeti ile inflamasyon belirteci CRP arasında ters bir korelasyon olduğunu ortaya koymuştur. Obezite, sigara kullanımı, kadın cinsiyet ve fiziksel aktivite azlığı gibi faktörler bu eksikliği tetiklemektedir. Oral D vitamini takviyesi alan hastalarda lezyonlarda ve nodül sayılarında belirgin klinik iyileşmeler kaydedilmiştir. Bu doğrultuda, rutin olarak tüm HS hastalarında D vitamini düzeylerinin izlenmesi ve eksiklik durumunda tedaviye başlanması önerilmektedir.
Hidradenitis Suppurativa (HS) is a chronic inflammatory skin disease characterized by painful nodules and fistulas that appear after puberty in areas dense with apocrine glands. In the pathogenesis of HS, where genetic and environmental factors play a role, defects in the innate immune system and skin barrier functions are observed. Recent studies have revealed that vitamin D deficiency is significantly higher in HS patients compared to the general population, and there is an inverse correlation between serum vitamin D levels, disease severity, and the inflammatory marker CRP. Factors such as obesity, smoking, female gender, and lack of physical activity trigger this deficiency. Significant clinical improvements in lesions and nodule counts have been recorded in patients receiving oral vitamin D supplementation. Accordingly, it is recommended to routinely monitor vitamin D levels in all HS patients and initiate treatment in case of deficiency.
