Seboreik Dermatit ve D Vitamini

Özet

Seboreik dermatit, özellikle saçlı deri, yüz ve üst gövde gibi yağ bezlerinin yoğun olduğu bölgeleri tutan, toplumda %5 sıklıkla görülen kronik inflamatuar bir deri hastalığıdır. Etyopatogenezinde artmış sebum, Malassezia mayaları ve bireysel immün yanıt rol oynamaktadır. Son araştırmalar, antienflamatuar, immünregülatör ve antioksidan etkinlikleri kanıtlanmış olan D vitamininin eksikliğinin bu hastalığın gelişiminde ve atakların tetiklenmesinde önemli bir payı olabileceğini göstermektedir. Yapılan çalışmalarda, seboreik dermatit hastalarının serum D vitamini düzeyleri genel olarak yetersiz veya eksik bulunmuş, vitamin seviyesi düştükçe hastalık şiddetinin arttığı gözlenmiştir. Tedavide geleneksel olarak antifungal ve antiinflamatuar ajanlar kullanılsa da D vitamini desteği ümit verici bir alternatif sunmaktadır. Klinik araştırmalar, oral D vitamini replasmanının inflamasyonu baskılayarak atak sıklığını belirgin şekilde azalttığını ortaya koymuştur. Topikal D vitamini analogları (takalsitol ve kalsipotriol) üzerine yapılan çalışmalarda ise özellikle saçlı deri ve dirençli lezyonlarda başarılı sonuçlar ve hızlı iyileşmeler rapor edilmiştir. Ancak, yüz yerleşimli lezyonlarda bazı çalışmalarda etkisiz olduğu da bildirilmiştir. Sonuç olarak, seboreik dermatit yönetiminde D vitamininin hem oral hem de topikal kullanımı hastalar için güçlü ve etkin bir tedavi seçeneği olarak öne çıkmaktadır.

Seborrheic dermatitis is a chronic inflammatory skin disease with a 5% prevalence in society, primarily affecting areas rich in sebaceous glands such as the scalp, face, and upper trunk. Increased sebum, Malassezia yeasts, and individual immune responses play pivotal roles in its etiopathogenesis. Recent studies suggest that a deficiency in vitamin D, which possesses proven anti-inflammatory, immunoregulatory, and antioxidant activities, may significantly contribute to disease development and trigger flare-ups. Research indicates that serum vitamin D levels are generally insufficient or deficient in seborrheic dermatitis patients, with disease severity increasing as vitamin levels decrease. Although antifungal and anti-inflammatory agents are traditionally utilized in treatment, vitamin D supplementation offers a promising alternative. Clinical trials have demonstrated that oral vitamin D replacement markedly reduces the frequency of attacks by suppressing inflammation. Furthermore, studies on topical vitamin D analogs, such as tacalcitol and calcipotriol, have reported successful outcomes and rapid healing, particularly in scalp involvement and treatment-resistant lesions. However, some studies have noted its ineffectiveness in facial lesions. Consequently, both oral and topical administration of vitamin D emerges as a potent and effective therapeutic option in managing seborrheic dermatitis.
