Nevüsler ve D Vitamini
Özet
Bu metin, nevüsler (benler) ile D vitamini arasındaki ilişkiyi etyopatojenez, serum düzeyleri ve tedavi alternatifleri açısından incelemektedir. Melanositik ve epidermal nevüsler iyi huylu tümörler veya hamartomlar olarak tanımlanırken, D vitamininin antiproliferatif, proapoptotik ve immünmodülatör etkileriyle bu lezyonların gelişimini etkileyebileceği belirtilmektedir. Yapılan çalışmalarda, nevüslerdeki vitamin D reseptörü (VDR) ekspresyonunun malign melanoma göre farklılık gösterdiği, tümör ilerledikçe VDR ifadesinin azaldığı saptanmıştır. Bazı araştırmalar serum D vitamini düzeyleri ile nevüs sayısı arasında pozitif korelasyon gösterirken, retrospektif analizler iyi huylu tümör grubunda düşük D vitamini seviyelerine işaret etmektedir. Tedavi boyutunda, melanositik nevüslerde cerrahi eksizyon birincil seçenek olsa da, kalsipotriol gibi sentetik D vitamini analoglarının özellikle inflamatuar lineer verrüköz epidermal nevüs (ILVEN) ve yaygın epidermal nevüs olgularında lezyonları geriletmede, kaşıntı ve eritemi azaltmada umut verici sonuçlar sağladığı bildirilmektedir. Sonuç olarak, D vitamininin nevüs patolojilerindeki rolü ve malign melanom kemoprofilaksisindeki potansiyeli gelecekteki daha geniş kapsamlı klinik çalışmalarla aydınlatılmayı beklemektedir.
This text examines the relationship between nevi (moles) and vitamin D in terms of etiopathogenesis, serum levels, and treatment alternatives. While melanocytic and epidermal nevi are defined as benign tumors or hamartomas, it is stated that vitamin D may influence the development of these lesions through its antiproliferative, proapoptotic, and immunomodulatory effects. Studies have determined that vitamin D receptor (VDR) expression in nevi differs compared to malignant melanoma, and VDR expression decreases as the tumor progresses. While some research shows a positive correlation between serum vitamin D levels and nevus count, retrospective analyses point to low vitamin D levels in the benign tumor category. Regarding treatment, although surgical excision is the primary option for melanocytic nevi, synthetic vitamin D analogs such as calcipotriol are reported to provide promising results in regressing lesions and reducing pruritus and erythema, particularly in cases of inflammatory linear verrucous epidermal nevus (ILVEN) and systematized epidermal nevus. In conclusion, the role of vitamin D in nevus pathologies and its potential in malignant melanoma chemoprofilaxis await elucidation through more extensive future clinical studies.
