Özet

D vitamini, kalsiyum-fosfor metabolizmasının yanı sıra vücutta 200'den fazla genin ekspresyonunu kontrol eden ve bağışıklık ile kardiyovasküler sistemler üzerinde önemli rolleri olan yağda çözünen bir vitamindir. Son yıllarda yapılan çalışmalar, D vitamini eksikliği ile venöz tromboz (dolaşım sisteminde kanın pıhtılaşması) arasında önemli bir ilişki olduğunu ortaya koymaktadır. Tromboz vakalarının kış aylarında artması ve derin ven trombozu olan hastalarda D vitamini seviyelerinin belirgin şekilde düşük saptanması bu ilişkiyi desteklemektedir. D vitamini; doku faktörünü azaltarak, trombomodulini artırarak ve inflamatuar sitokinlerin salınımını baskılayarak antitrombotik etki gösterir, böylece damar endotel bütünlüğünü korur. Eksikliği durumunda ise insülin direnci, oksidatif stres ve inflamasyon gibi mekanizmalar tetiklenerek pıhtılaşma riski artar. Özellikle venöz tromboz riski taşıyan bireylerde D vitamini replasman tedavisi (sabah saatlerinde ve yüksek dozda) pıhtı riskini azaltmada etkilidir. Sentetik takviyeler yerine güneş ışığı maruziyeti ve Akdeniz diyeti gibi doğal yollarla D vitamini düzeylerinin artırılması önerilmektedir.

Vitamin D is a fat-soluble vitamin that regulates calcium-phosphorus metabolism and controls the expression of more than 200 genes, playing significant roles in the immune and cardiovascular systems. Recent studies demonstrate a strong correlation between vitamin D deficiency and venous thrombosis, which is characterized by blood clotting within the circulatory system. This relationship is supported by the increased incidence of thrombotic events during winter months and the remarkably low vitamin D levels observed in deep vein thrombosis patients. Vitamin D exerts antithrombotic effects by decreasing tissue factor, increasing thrombomodulin, and suppressing the release of inflammatory cytokines, thereby maintaining vascular endothelial integrity. Conversely, its deficiency triggers mechanisms such as insulin resistance, oxidative stress, and inflammation, which elevate the risk of clotting. Vitamin D replacement therapy, particularly administered in high doses during morning hours, is effective in mitigating clot risks for individuals predisposed to venous thrombosis. Rather than relying solely on synthetic supplements, increasing vitamin D levels through natural means, such as enhanced sun exposure and a Mediterranean diet, is highly recommended.
