Özet

Yara iyileşmesi; inflamasyon, yeni doku oluşumu ve yeniden şekillenme aşamalarından oluşan karmaşık bir biyolojik süreçtir. D vitamininin bu süreçteki rolü tam açıklığa kavuşmamış olsa da anti-inflamatuar, anti-proliferatif ve antimikrobiyal özellikleri ile iyileşmeye katkı sağladığı bilinmektedir. 1,25(OH)2D3; makrofajların kemotaktik ve fagositik tepkilerini artırırken, proinflamatuar sitokinleri baskılamakta ve yara iyileşmesini destekleyen Th-2 sitokinlerinin üretimini uyarmaktadır. Ayrıca epidermisteki D vitamini reseptörleri (VDR), keratinosit farklılaşmasını destekleyerek epidermal bariyer oluşumunu düzenler. Klinik çalışmalarda; yanık yaraları, venöz ülserler, bası ülserleri ve diyabetik ayak ülserleri olan hastalarda D vitamini eksikliğinin yüksek oranlarda görüldüğü; düşük D vitamini seviyelerinin yara iyileşme süresini uzattığı ve yara alanını genişlettiği saptanmıştır. Hayvan modellerinde ve in vitro çalışmalarda, topikal ve sistemik D vitamini takviyelerinin, kalsipotriolün ve fibronektin sentezinin uyarılmasının yara kapanmasını hızlandırdığı, enflematuar yanıtı azalttığı ve iyileşmeyi desteklediği gösterilmiştir. Sonuç olarak, D vitamininin yara iyileşmesinin farklı aşamalarında kritik rolleri bulunmakta olup kronik yaraların tedavisinde alternatif bir seçenek olarak kullanılabilmesi için daha fazla klinik çalışmaya, doz ve süre belirlenmesine ihtiyaç duyulmaktadır.

Wound healing is a complex biological process consisting of inflammation, new tissue formation, and remodeling phases. Although the exact role of vitamin D in this process is not fully elucidated, it is known to contribute to healing through its anti-inflammatory, anti-proliferative, and antimicrobial properties. 1,25(OH)2D3 enhances the chemotactic and phagocytic responses of macrophages, suppresses pro-inflammatory cytokines, and stimulates the production of Th-2 cytokines that support wound healing. Additionally, vitamin D receptors (VDR) in the epidermis regulate epidermal barrier formation by promoting keratinocyte differentiation. Clinical studies have revealed high rates of vitamin D deficiency in patients with burn wounds, venous ulcers, pressure ulcers, and diabetic foot ulcers, showing a negative correlation where low vitamin D levels prolong healing time and increase wound size. In animal models and in vitro studies, topical and systemic vitamin D supplementation, calcipotriol, and the stimulation of fibronectin synthesis have been shown to accelerate wound closure, reduce the inflammatory response, and promote healing. Consequently, while vitamin D plays critical roles in various stages of wound healing, more studies are required to determine its efficacy, dosage, and duration for alternative use in chronic wound treatment.
