Özet

Vitamin D, kemik sağlığının ötesinde deri enfeksiyonları ve bağışıklık sistemi üzerinde de kritik rollere sahiptir. Temel olarak dermiste sentezlenen bu vitamin, keratinosit fonksiyonlarını düzenleyerek ve katelisidin ile beta defensin gibi antimikrobiyal peptitlerin ekspresyonunu artırarak epidermal bariyeri güçlendirir. Eksikliği veya yetmezliği durumunda, derinin savunma mekanizmaları zayıflayarak viral, bakteriyel ve fungal enfeksiyonlara karşı duyarlılık artar. Klinik çalışmalar; COVID-19 seyrindeki proinflamatuar sitokinlerin ve sepsis riskinin azaltılmasında, Hepatit B ve C virüs replikasyonunun baskılanmasında, HIV hastalarında CD4+ düzeylerinin desteklenmesinde vitamin D'nin olumlu etkilerini göstermiştir. Ayrıca HPV'ye bağlı dirençli siğillerin tedavisinde, Herpes zoster sonrası nöral iyileşmede ve yaygın tinea korporis ile kandidal mantar enfeksiyonlarında vitamin D düzeylerinin doğrudan ilişkili olduğu ve destek tedavisinin başarıyı artırdığı saptanmıştır. Mikobakteriyel enfeksiyonlardan lepra ve deri tüberkülozunda da vitamin D seviyeleri ile bakteriyel indeks arasında paralellik bulunmaktadır. Sonuç olarak, ciddi vitamin D eksikliği akut ve kronik deri enfeksiyonu riskini doğrudan artırmakta olup, bu alanda daha geniş klinik çalışmalara ihtiyaç duyulmaktadır.

Vitamin D plays a critical role in skin infections and the immune system beyond bone health. Synthesized primarily in the dermis, this vitamin regulates keratinocyte functions and strengthens the epidermal barrier by increasing the expression of antimicrobial peptides such as cathelicidin and beta-defensin. In cases of deficiency or insufficiency, the skin's defense mechanisms weaken, leading to increased susceptibility to viral, bacterial, and fungal infections. Clinical studies have demonstrated the positive effects of vitamin D in reducing proinflammatory cytokines and the risk of sepsis in the course of COVID-19, suppressing Hepatitis B and C virus replication, and supporting CD4+ levels in HIV patients. Furthermore, it has been determined that vitamin D levels are directly correlated with and that supportive treatment increases success in the management of recalcitrant warts linked to HPV, neural healing following Herpes zoster, and extensive tinea corporis as well as candidal fungal infections. A parallelism also exists between vitamin D levels and the bacterial index in mycobacterial infections, including leprosy and skin tuberculosis. In conclusion, severe vitamin D deficiency directly increases the risk of acute and chronic skin infections, and more extensive clinical studies are still required in this field.
