Özet

D vitamini, kalsiyum-fosfat dengesini ve kemik metabolizmasını düzenlemenin ötesinde; immünomodülatör, anti-inflamatuar, anti-enfektif ve anti-kanserojen etkileriyle oral mukoza sağlığında kritik roller oynamaktadır. Vücuttaki aktif formu olan kalsitriol, immün sistem hücreleri ve keratinositlerde yaygın olarak bulunan D vitamini reseptörleri (VDR) aracılığıyla hücresel ve hümöral immün yanıtı modüle eder. Araştırmalar; rekkürent aftöz stomatit (RAS), Behçet hastalığı, PFAPA sendromu, Sjögren sendromu ve pemfigus vulgaris gibi dermatolojik ve otoimmün mukoza hastalıklarında serum D vitamini düzeylerinin sağlıklı bireylere göre anlamlı derecede düşük olduğunu ve bu eksikliğin hastalıkların patogenezi ve şiddetiyle ilişkili olduğunu göstermektedir. D vitamini, Th1 sitokin üretimini baskılayıp Th2 sitokinlerini artırarak otoimmünite riskini azaltırken, Toll-like reseptörler üzerinden kathelisidin ve defensin gibi antibakteriyel proteinlerin sentezini uyararak enfeksiyonlara karşı doğal savunmayı güçlendirir. Ayrıca, hücre proliferasyonunu inhibe etme ve apoptozu indükleme yeteneği sayesinde oral liken planus gibi pre-kanseröz lezyonlar ile skuamöz hücreli karsinom gibi oral kanserlerin önlenmesinde ve tedavisinde ümit verici destekleyici etkiler sunmaktadır. Sonuç olarak, oral mukozanın inflamatuar, enfektif ve neoplastik süreçlerinde D vitamini eksikliğinin giderilmesi ve topikal ya da sistemik takviyelerin klinik tedavilere eklenmesi, terapötik başarıyı ve hastaların yaşam kalitesini artırmada önemli bir potansiyele sahiptir.

Vitamin D plays critical roles in oral mucosal health through its immunomodulatory, anti-inflammatory, anti-infective, and anti-carcinogenic effects, extending far beyond its primary function of regulating calcium-phosphate balance and bone metabolism. Calcitriol, the active form in the body, modulates cellular and humoral immune responses via vitamin D receptors (VDR) widely expressed in immune system cells and keratinocytes. Research indicates that serum vitamin D levels are significantly lower in patients with dermatological and autoimmune mucosal diseases—such as recurrent aphthous stomatitis (RAS), Behçet's disease, PFAPA syndrome, Sjögren's syndrome, and pemphigus vulgaris—compared to healthy controls, linking this deficiency to disease pathogenesis and severity. By suppressing Th1 cytokine production and increasing Th2 cytokines, vitamin D mitigates autoimmunity risks, while simultaneously reinforcing innate defense against infections by stimulating the synthesis of antibacterial proteins like cathelicidins and defensins via Toll-like receptors. Furthermore, owing to its capacity to inhibit cell proliferation and induce apoptosis, it offers promising supportive effects in preventing and treating pre-cancerous lesions like oral lichen planus, as well as oral malignancies such as squamous cell carcinoma. Consequently, addressing vitamin D deficiency and integrating topical or systemic supplementation into clinical protocols holds substantial potential for enhancing therapeutic success and improving patient quality of life across inflammatory, infective, and neoplastic oral mucosal conditions.
