Baş Ağrılı Hastaya Yaklaşım, Öykü, Muayene ve İnceleme Yöntemleri
Özet
Baş ağrısı, dünya genelinde hastaneye başvurularda en sık karşılaşılan ve klinisyenler için hayati önem taşıyan yaygın semptomlardan biridir. Doğru tanı ve tedavi planlaması için ilk ve en kritik adım, hastadan veya yakınından alınan ayrıntılı bir öykü ile kapsamlı bir fizik ve nörolojik muayenedir. Uluslararası Baş Ağrısı Sınıflaması'na (ICHD-3) göre baş ağrıları; primer, sekonder ve ağrılı kranial nöropatiler olmak üzere üç ana grupta toplanır. Vakalardan elde edilen verilerin yaklaşık %90'ı altta yatan başka bir nedenin bulunmadığı migren veya gerilim tipi gibi primer baş ağrılarından oluşur. Geriye kalan sekonder baş ağrıları ise travma, tümör, menenjit, intrakranial kanama veya ilaç aşırı kullanımı gibi ciddi ve acil müdahale gerektiren hayati risk altındaki patolojilere bağlı olarak gelişen ikincil semptomlardır. Tanı aşamasında 50 yaş üstü başlangıç, ense sertliği, papil ödem veya fokal nörolojik defisit gibi "kırmızı bayrak" olarak nitelendirilen tehlike işaretlerinin varlığı klinisyeni doğrudan sekonder nedenlere yönlendirmelidir. Bu gibi riskli ve atipik durumlarda sekonder nedenleri ekarte etmek veya doğrulamak amacıyla kontrastsız beyin tomografisi, kranial MR, MR anjiyografi ve lomber ponksiyon gibi ileri laboratuvar ve nörogörüntüleme inceleme yöntemlerine başvurulması hayati bir gerekliliktir.
Headache is one of the most common symptoms causing hospital admissions worldwide, and obtaining a detailed history and performing a comprehensive physical and neurological examination constitutes the first and most critical step for correct diagnosis and treatment planning. According to the International Headache Classification (ICHD-3), headaches are categorized into three main groups as primary, secondary, and painful cranial neuropathies, with primary headaches, such as migraine or tension-type, accounting for approximately 90% of cases where the headache itself is the main disease without any underlying cause. Secondary headaches, on the other hand, appear as secondary symptoms due to underlying pathologies that require urgent intervention and carry vital risks, including trauma, tumors, meningitis, intracranial hemorrhages, or medication overuse. During the diagnostic phase, the presence of danger signs characterized as "red flags," such as onset over the age of 50, neck stiffness, papilledema, or focal neurological deficits, should directly alert the clinician toward secondary etiologies. In such risky and atypical situations, utilizing advanced laboratory and neuroimaging methods, including non-contrast brain computed tomography, cranial MR, MR angiography, and lumbar puncture, is a vital necessity to exclude or confirm secondary causes.
