Özet

Medikal tedavilerin yetersiz kaldığı primer ve sekonder baş ağrılarının (migren, küme, ilaç aşırı kullanımı, postdural ponksiyon baş ağrıları ve nevraljiler) tedavisinde uygulanan periferik sinir blokajları, hem akut hem de koruyucu tedavi seçenekleri sunmaktadır. Bu yöntemler, trigeminovasküler sistem ve santral ağrı modülasyonu üzerinde etki göstererek uzun süreli ağrı kontrolü sağlamaktadır. En yaygın uygulama büyük oksipital sinir (GON) blokajı olup, bunun yanı sıra sfenopalatin gangliyon (SPG), supraorbital, infraorbital, aurikülotemporal ve mental sinir blokajları da mevcuttur. Blokajlarda genellikle alerji riski düşük olan amid yapılı lokal anestezikler tercih edilirken, steroid kombinasyonlarının ek bir fayda sağlamadığı ve yan etki riskini artırdığı belirtilmektedir. Periferik sinir blokajları; ilaç kullanamayan, böbrek veya karaciğer hastalığı olan hastalar ile hamileler ve çocuklar için güvenli, yan etkisi az ve maliyeti düşük bir alternatiftir. Tekrarlayan uygulamalarla maksimum etkinliğe ulaşılabilen bu yöntemler, günlük pratikte önemli bir yere sahiptir.

Peripheral nerve blockades applied in the treatment of primary and secondary headaches—such as migraine, cluster headache, medication overuse headache, postdural puncture headache, and neuralgias—where medical treatments prove insufficient offer both acute and preventive treatment options. These methods provide long-term pain control by acting on the trigeminovascular system and central pain modulation. The most common application is the greater occipital nerve (GON) block, alongside other techniques including sphenopalatin ganglion (SPG), supraorbital, infraorbital, auriculotemporal, and mental nerve blocks. Amide-type local anesthetics with a low risk of allergy are generally preferred in blockades, whereas steroid combinations are reported to offer no extra benefit while increasing the risk of side effects. Peripheral nerve blocks serve as a safe, low-cost alternative with minimal side effects for patients who cannot use standard medications, those with kidney or liver diseases, as well as pregnant women and children. These methods, through which maximum efficacy can be achieved via repeated applications, hold a significant place in daily clinical practice.
