Baş Ağrısı Tedavisinde Geleneksel ve Tamamlayıcı Tıp

Özet

Baş ağrısı, dünya genelinde yüksek prevalansa sahip, iş gücü ve ekonomik kayıplara yol açan önemli bir sağlık sorunudur. Uluslararası Baş Ağrısı Topluluğu'na göre primer baş ağrıları (migren, gerilim tipi baş ağrısı vb.) en sık görülen grubu oluşturmaktadır. Son yıllarda geleneksel ve tamamlayıcı tıp (GETAT) uygulamaları, baş ağrısı tedavisinde yaygınlaşmış ve Türkiye'de 2014 yılında yayımlanan yönetmelikle resmi bir altyapıya kavuşmuştur. Bu kapsamda akupunktur, migren ve gerilim tipi baş ağrılarında atak sıklığını azaltmada ve maliyet etkin bir çözüm sunmada kanıtlanmış etkilere sahiptir. Benzer şekilde fitoterapi, hipnoz, homeopati, kupa tedavisi, kayropraktik tedavi, mezoterapi ve refleksoloji gibi yöntemlerin de baş ağrısı yoğunluğunu, süresini ve atak sıklığını azalttığı klinik çalışmalarla gösterilmiştir. Diğer taraftan apiterapi, osteopati ve proloterapi alanlarında daha geniş kapsamlı araştırmalara ihtiyaç duyulurken; larva tedavisinin baş ağrısında endikasyonu bulunmamaktadır. Ozon tedavisi gibi bazı uygulamaların ise pnömosefali gibi ciddi yan etki riskleri mevcuttur. Sonuç olarak, baş ağrısı tedavisinde sosyal huzurun ve fonksiyonelliğin artırılması amacıyla, hastaya özel olarak seçilen klasik ve GETAT yöntemlerinin kombinasyonu içeren sonuç odaklı bir yaklaşım benimsenmelidir.

Headache is a major health problem worldwide with a high prevalence, leading to workforce and economic losses. According to the International Headache Society, primary headaches (migraine, tension-type headache, etc.) constitute the most common group. In recent years, traditional and complementary medicine (TCM) practices have become widespread in headache treatment and achieved an official framework with a regulation published in Turkey in 2014. Within this scope, acupuncture has proven effects in reducing attack frequency in migraine and tension-type headaches and offers a cost-effective solution. Similarly, clinical studies have demonstrated that methods such as phytotherapy, hypnosis, homeopathy, cupping therapy, chiropractic treatment, mesotherapy, and reflexology also decrease headache intensity, duration, and attack frequency. On the other hand, while more comprehensive research is needed in apitherapy, osteopathy, and prolotherapy, larva therapy has no indication for headaches. Some applications, such as ozone therapy, carry risks of serious side effects like pneumocephalus. Consequently, to enhance social well-being and functionality in headache management, a result-oriented approach incorporating a patient-specific combination of conventional and TCM methods should be adopted.
