RUH SAĞLIĞI HEMŞİRELİĞİNDE ENFEKSİYON KONTROLÜ

Özet

Ruh sağlığı hemşireliğinde enfeksiyon kontrolü, psikiyatri servislerinin kalabalık yapısı, ortak yaşam alanları ve hastaların öz bakım yetersizlikleri nedeniyle büyük önem taşımaktadır. Psikiyatri kliniklerinde yatan hastalar; iç görü eksikliği, antipsikotik ilaç kullanımı ve hareketsiz yaşam tarzı gibi faktörler yüzünden enfeksiyonlara karşı daha savunmasızdır. Bu birimlerde idrar yolu, alt solunum yolu ve cilt enfeksiyonları sık görülmekte, norovirüs gibi viral patojenler salgınlara yol açabilmektedir. Enfeksiyon zincirini kırmak adına el hijyeni ve kişisel koruyucu ekipman kullanımı gibi standart önlemler hayati müdahalelerdir. Ancak, temizlik solüsyonlarının hastaların kendine zarar verme riski nedeniyle kısıtlanması ve izolasyonun hastalar üzerinde yarattığı olumsuz psikolojik etkiler, bu süreçte ek zorluklar oluşturmaktadır. Ayrıca şizofreni hastalarında antipsikotiklere bağlı gelişen ağız kuruluğu ile agranülositoz gibi yan etkiler yakından takip edilmelidir. Kan ve cinsel yolla bulaşan HIV ve Hepatit gibi hastalıklar açısından riskli davranışlar gösteren hastalar için de özel danışmanlık süreçleri yürütülmelidir. Sonuç olarak, psikiyatri hemşirelerinin hem terapötik ortamın güvenliğini sağlaması hem de personelin düzenli eğitimlerle enfeksiyon yönetim becerilerini artırması kritik bir gerekliliktir.

Infection control in mental health nursing is of paramount importance due to the crowded nature of psychiatric wards, shared living spaces, and patients' self-care deficits. Inpatients in psychiatric clinics are more vulnerable to infections due to factors such as lack of insight, antipsychotic drug use, and sedentary lifestyles. Urinary tract, lower respiratory tract, and skin infections are commonly observed in these units, and viral pathogens like norovirus can cause outbreaks. Standard precautions, such as hand hygiene and the use of personal protective equipment, are vital interventions to break the chain of infection. However, restricting cleaning solutions due to patients' self-harm risks and the negative psychological impacts of isolation on patients create additional challenges in this process. Furthermore, side effects such as dry mouth and agranulocytosis caused by antipsychotics in schizophrenia patients must be closely monitored. Specialized counseling processes should also be conducted for patients who exhibit risky behaviors regarding blood-borne and sexually transmitted diseases like HIV and Hepatitis. Consequently, it is a critical necessity for psychiatric nurses to ensure the safety of the therapeutic environment and to enhance the infection management skills of the staff through regular training.
