DİYALİZ HEMŞİRELİĞİNDE ENFEKSİYON KONTROLÜ VE ÇOCUK
Özet
Kronik böbrek yetmezliği olan çocuk ve yetişkin hastalarda diyaliz tedavisi sürecinde enfeksiyon kontrolü, mortalite ve morbidite oranlarını düşürmek adına hayati bir önem taşımaktadır. Böbrek hastalıklarında uygulanan renal replasman tedavileri yaşam süresini uzatırken, zayıflamış bağışıklık sistemi ve sık tekrarlanan invaziv girişimler nedeniyle enfeksiyon duyarlılığını ciddi şekilde artırmaktadır. Yetişkinlerde hemodiyaliz ön plana çıkarken, çocuklarda sıklıkla periton diyalizi tercih edilmekte ve bu grupta en sık komplikasyon olarak peritonit ile karşılaşılmaktadır. Kateter ilişkili bakteriyemi ve sepsise yol açan MRSA gibi bakteriyel etkenlerin yanı sıra kan yoluyla bulaşan HBV, HCV ve HIV gibi viral enfeksiyonlar diyaliz ünitelerinde yüksek risk oluşturmaktadır. Çapraz bulaşmayı önlemek amacıyla el antisepsisi, istasyon değişimi kuralları, koruyucu ekipman kullanımı ve diyaliz makinelerinin klor bazlı veya klor dışı gün sonu dezenfektanlarıyla arındırılması gibi standart kontrol önlemlerinin eksiksiz uygulanması gerekmektedir. Kan önlemleri kapsamında hastaların serolojik takipleri düzenli yapılmalı, hepatit taramaları gerçekleştirilmeli ve aşılama programları çocuk hastalar dahil olmak üzere kişiye özel planlanmalıdır. Sağlık personeli, hasta ve bakım verenlerin düzenli eğitimiyle desteklenen bu protokoller, diyaliz süreçlerinde güvenliği sağlamanın en etkili yoludur.
Infection control during dialysis treatment for pediatric and adult patients with chronic kidney failure is critical to reducing mortality and morbidity rates. While renal replacement therapies extend life expectancy, the immunosuppressed status of patients combined with frequent invasive procedures significantly heightens susceptibility to infections. While hemodialysis is prominent in adults, peritoneal dialysis is commonly preferred in children, with peritonitis emerging as the most frequent complication in pediatric cases. In addition to bacterial pathogens such as MRSA that trigger catheter-related bacteremia and sepsis, blood-borne viral infections including HBV, HCV, and HIV pose major risks in dialysis units. To prevent cross-contamination, strict adherence to standard control measures—such as hand antisepsis, proper station turnover protocols, use of protective equipment, and meticulous disinfection of dialysis machines with chlorine-based or end-of-day disinfectants—is mandatory. Blood-borne precaution protocols require regular serological monitoring, hepatitis screening, and customized vaccination programs implemented for each patient, including children. Comprehensive infection control strategies, reinforced by continuous education for healthcare professionals, patients, and caregivers, remain the most effective method to ensure safety in dialysis procedures.
