ONKOLOJİ HEMŞİRELİĞİNDE ENFEKSİYON KONTROLÜ

Özet

Kanser tedavisi gören hastalar, tümörün hayati organlara yayılmasının yanı sıra hastalık sürecinde ve miyelosupresyon gibi tedavinin getirdiği yan etkiler sonucunda gelişen enfeksiyonlar nedeniyle hayatlarını kaybetme riskiyle karşı karşıyadır. Bu bağlamda, onkoloji hemşireliğinde enfeksiyon kontrol stratejileri, modern onkolojik bakımın temel bir parçası olup hasta güvenliğini ve bakım kalitesini artırırken finansal yükü azaltmaktadır. Enfeksiyonların önlenmesinde el hijyeni, Dünya Sağlık Örgütü'nün "El Hijyeni İçin Beş An" kampanyası çerçevesinde en temel ve kritik basamağı oluşturmaktadır. Ayrıca, yüksek riskli hematopoetik kök hücre nakli ünitelerinde hepafiltreli pozitif basınçlı odaların kullanımı, çevre temizliği, su sistemlerinin kontrolü ve inşaat alanlarının izole edilmesi enfeksiyon gelişimini önlemede büyük rol oynar. Çok ilaca dirençli mikroorganizmalar ve solunum yolu virüsleri ile mücadelede izolasyon tedbirleri, maske kullanımı ve personelin aşılanması hayati önem taşımaktadır. Kateter ilişkili enfeksiyonların takibi ve standart besin hijyeni ilkelerine uyulması da sürecin diğer önemli parçalarıdır. Tedavilerin sıklıkla ayaktan yürütülmesi sebebiyle; el yıkama, cilt, ağız, gastrointestinal ve genitoüriner sistem korumasını içeren kapsamlı hasta ve aile eğitimi başarının anahtarıdır. Sonuç olarak, özelleşmiş onkoloji hemşirelerinin yürüttüğü multidisipliner ekip yaklaşımı ve düzenli rehberlik, enfeksiyonları önleyerek kanser tedavilerinin zamanında uygulanmasını ve etkinliğini sağlar.

Cancer patients face significant risks of mortality not only from the disease itself but also from infections arising due to immunosuppression and myelosuppression caused by various therapies. Consequently, infection control strategies in oncology nursing constitute a fundamental component of modern oncological care, enhancing patient safety, care quality, and reducing the overall financial burden. Hand hygiene serves as the most basic and critical step in preventing infections, strictly evaluated within the framework of the World Health Organization's "Five Moments for Hand Hygiene" campaign. Furthermore, in high-risk environments like hematopoietic stem cell transplant units, implementing protective measures such as HEPA-filtered, positive-pressure rooms, environmental cleaning, water system monitoring, and isolating construction zones is crucial to prevent pathogen transmission. Managing multidrug-resistant microorganisms and seasonal respiratory viruses requires strict isolation precautions, universal mask usage, and healthcare worker vaccination programs. Preventing catheter-associated infections and adhering to standard food safety principles are also vital pillars of this process. Since oncology treatments are predominantly administered in outpatient settings, exhaustive patient and family education covering handwashing, skin protection, oral hygiene, gastrointestinal care, and genitourinary protection is essential. Ultimately, a multidisciplinary team approach led by specialized oncology nurses ensures timely treatment delivery and optimal therapeutic efficacy through rigorous infection management.
