STOMA VE YARA BAKIM HEMŞİRELİĞİNDE ENFEKSİYON KONTROLÜ
Özet
Açık yara ve stoma bakımı, endojen ve ekzojen mikroorganizmaların neden olabileceği enfeksiyon riskleri nedeniyle yüksek klinik öneme sahiptir. Deri bütünlüğünün bozulmasıyla başlayan süreç; kontaminasyon, kolonizasyon ve lokal enfeksiyondan sistemik sepsise kadar ilerleyebilmektedir. Enfeksiyon gelişiminde mikroorganizma virülansı, bakteri yükü, lokal doku perfüzyonu ve immün sistem gibi faktörler belirleyici rol oynar. Doğru tanı için antibiyotik tedavisi öncesinde derin dokudan mikrobiyolojik örnek alınması esastır. Akut yaralarda ilk altı saatlik "altın süre" içinde serum fizyolojik ile irrigasyon yapılması, enfeksiyonu önlemede antibiyotiklerden daha etkilidir. Peristomal alanda ise barsak içerikleri ve kimyasal temizleyiciler irritan dermatit, mantar enfeksiyonları ve folikülite zemin hazırlar. Stoma ve yara bakım hemşireleri, aseptik dokunulmayan teknik (ANTT) kullanarak çapraz kontaminasyonu önler ve hidrofilik yara örtüleriyle nem dengesini korur. Ayrıca glisemik kontrol, dengeli beslenme ve sigarayı bırakma gibi sistemik faktörleri yöneterek hastaya holistik bir bakım sunar. Multidisipliner bir yaklaşımla yürütülen bu süreç, hastane enfeksiyon oranlarını azaltarak yaşam kalitesini artırır.
Open wound and stoma care hold high clinical significance due to infection risks posed by endogenous and exogenous microorganisms. The process initiated by the disruption of skin integrity can progress from contamination, colonization, and local infection to systemic sepsis. Factors such as microorganism virulence, bacterial load, local tissue perfusion, and the immune system play a decisive role in infection development. For accurate diagnosis, obtaining microbiological samples from deep tissue prior to antibiotic therapy is essential. In acute wounds, irrigation with saline solution within the "golden hours" of the first six hours is more effective than antibiotics in preventing infection. In the peristomal area, exposure to intestinal contents and chemical cleansers paves the way for irritant dermatitis, fungal infections, and folliculitis. Stoma and wound care nurses prevent cross-contamination by using the aseptic non-touch technique (ANTT) and maintain moisture balance with hydrophilic dressings. Furthermore, they provide holistic care by managing systemic factors such as glycemic control, balanced nutrition, and smoking cessation. This process, managed through a multidisciplinary approach, enhances patient quality of life by reducing nosocomial infection rates.
