SAĞLIK HİZMETİ İLİŞKİLİ ENFEKSİYONLARDA TIBBİ MİKROBİYOLOJİ LABORATUVARININ ROLÜ
Özet
Sağlık hizmeti ile ilişkili enfeksiyonlar (SHİE), yüksek mortalite ve maliyete yol açan ciddi komplikasyonlardır. Tıbbi mikrobiyoloji laboratuvarı, bu patojenlerin hızlı ve doğru tanımlanmasını sağlayarak enfeksiyon kontrol komitelerinin (EKK) en kritik bileşenlerinden birini oluşturur. Laboratuvarın temel görevleri arasında etkenlerin preanalitik, analitik ve postanalitik süreçlerde doğru izolasyonu, antibiyotik duyarlılık kalıplarının belirlenmesi, kısıtlı bildirim kurallarına göre raporlama yapılması ve izolatların arşivlenmesi yer alır. Özellikle beyin omurilik sıvısı (BOS) ve kan kültürü gibi steril vücut sıvılarında saptanan mikroorganizmalar ile MRSA, VRE gibi dirençli patojenlerin varlığında kliniklere derhal ön bildirim yapılması hayat kurtarıcıdır. Sürveyans ve salgın araştırmalarında laboratuvar, olası salgınların erken tespitini sağlar ve kaynak tespiti için personel ile çevreden alınan örnekleri değerlendirir. Mikroorganizmalar arasındaki klonal ilişkinin ve genetik akrabalık bağlarının saptanmasında, altın standart olan PFGE ve PCR gibi genotipik tiplendirme yöntemleri, geleneksel fenotipik yöntemlere göre daha yüksek ayrım gücü ve hız sunduğu için tercih edilmektedir. Sonuç olarak mikrobiyoloji laboratuvarı, SHİE'lerin önlenmesi, akılcı antibiyotik kullanımı ve etkili salgın yönetiminde anahtar rol oynamaktadır.
Healthcare-associated infections (HAIs) are critical complications leading to high mortality rates and increased treatment costs. The medical microbiology laboratory plays a pivotal role in the surveillance and control of HAIs by ensuring the rapid and accurate identification of pathogens. The primary responsibilities of the laboratory include the precise isolation of causative agents across preanalytical, analytical, and postanalytical stages, determining antimicrobial susceptibility patterns, and reporting findings based on selective reporting rules to prevent resistance development. Immediate notification of critical results, such as pathogen isolation from sterile body fluids like CSF and blood, or the detection of resistant strains like MRSA and VRE, is vital for timely intervention. In surveillance and outbreak investigations, the laboratory works in coordination with infection control committees to detect clusters early and identify sources through environmental and staff sampling. For epidemiological typing, genotypic methods such as PCR and the gold-standard PFGE are preferred over conventional phenotypic methods due to their higher discriminatory power, speed, and reproducibility in demonstrating genetic relationships among isolates. Ultimately, the microbiology laboratory serves as a cornerstone in preventing HAIs, guiding rational antibiotic therapy, and managing hospital outbreaks effectively.
