Özet

Sağlık hizmeti ilişkili enfeksiyonlar (SHİE), morbidite ve mortaliteyi artıran, hastanede kalış sürelerini uzatan ve küresel sağlık sistemlerine ciddi mali yükler getiren en önemli hasta güvenliği sorunlarından biridir. Mikroorganizmaların hastalar arasındaki en yaygın bulaş yolu sağlık çalışanlarının kontamine elleridir. Tarihsel süreçte Alexander Gordon, Oliver Wendell Holmes ve özellikle Ignaz Semmelweis gibi öncüler, el hijyeninin loğusalık humması ve enfeksiyonları önlemedeki kritik rolünü ortaya koymuşlardır. Günümüzde Dünya Sağlık Örgütü (DSÖ), el temizliği uygulamalarını kolaylaştırmak amacıyla "HAYAT KURTARIN: Ellerinizi Temizleyin" kampanyası altında "El Hijyeninde 5 Endikasyon" yaklaşımını geliştirmiştir. Bu yaklaşım; hastayla temastan önce, temiz/aseptik işlemden önce, vücut sıvısıyla maruziyet riski sonrası, hastayla temastan sonra ve hasta çevresiyle temastan sonra el hijyeni uygulanmasını zorunlu kılar. Ayrıca, hastanelerde el hijyenine uyumu artırmak için sistem değişimi, eğitim, değerlendirme ve geribildirim, hatırlatıcılar ve kurumsal güvenlik iklimini içeren çok bileşenli bir strateji önerilmektedir. Bu süreçte, hastalarla en sık temas eden grup olan hemşireler, kurumsal güvenliğin sağlanmasında ve çapraz kontaminasyonun önlenmesinde hayati bir role sahiptir.

Healthcare-associated infections (HAIs) represent a major patient safety challenge globally, contributing to increased morbidity, mortality, and extended hospital stays while imposing substantial financial burdens on healthcare systems. The most common vehicle for the transmission of pathogens between patients is the contaminated hands of healthcare workers. Historically, pioneers such as Alexander Gordon, Oliver Wendell Holmes, and notably Ignaz Semmelweis demonstrated the critical impact of hand hygiene in preventing childbed fever and clinical infections. To standardize compliance, the World Health Organization (WHO) introduced the "My 5 Moments for Hand Hygiene" approach under the "SAVE LIVES: Clean Your Hands" campaign. This framework dictates performing hand hygiene before touching a patient, before clean/aseptic procedures, after body fluid exposure risk, after touching a patient, and after touching patient surroundings. Furthermore, a multimodal improvement strategy comprising system change, continuous education, evaluation with feedback, workplace reminders, and an institutional safety climate is recommended to enhance compliance. Given their high density of patient contact, nurses play a paramount role in fostering this safety culture, monitoring resources, and successfully breaking the chain of cross-contamination within healthcare facilities.
