Puerperium
Özet
Puerperium, doğum sonrası plasentanın ayrılmasıyla başlayıp maternal fizyolojik değişikliklerin gebelik öncesi duruma dönmesini kapsayan ve genellikle 6 hafta süren bir periyottur. Bu süreçte uterus, fundus seviyesinin küçülmesi ve myometrial kontraksiyonlar yardımıyla hızla involusyona uğrar; endometrium epiteli yenilenirken löchia adı verilen akıntılar gözlenir. Serviks kademeli olarak kapanır, vajina ve perineal yaralanmalar belirli sürelerde iyileşir ve pelvik taban kasları tonusunu yeniden kazanır. Meme dokusunda gebelikle başlayan proliferasyon ve hormonal etkilerle doğum sonrası laktasyon süreci başlar; ilk günlerde kolostrum, ardından normal süt sekresyonu gerçekleşir. Genel sistemik düzeyde ise üriner sistemde diürez artar, kardiyovasküler sistem, akciğer hacimleri ve endokrin fonksiyonlar zamanla eski haline döner. Postpartum bakım; erken mobilizasyon, uygun diyet, emzirmenin teşviki, hijyen ve %70 oranında görülebilen geçici postpartum hüzün tablosunun takibini içerir. Bu dönemin önemli komplikasyonları arasında puerperal ateş, endometrit, yara ayrılması, üriner enfeksiyonlar ile meme angorjmanı, mastit ve meme apsesi gibi laktasyon sorunları yer almaktadır.
Puerperium is a period that begins with the separation of the placenta after birth, encompasses the return of maternal physiological changes to the pre-pregnancy state, and generally lasts for 6 weeks. During this process, the uterus undergoes rapid involution with the help of myometrial contractions and the shrinking of the fundus level, while the endometrium epithelium regenerates and discharges called lochia are observed. The cervix gradually closes, vaginal and perineal injuries heal within certain periods, and pelvic floor muscles regain their tone. With the proliferation starting in pregnancy and hormonal effects, the postpartum lactation process begins in the breast tissue; colostrum is produced in the first days, followed by normal milk secretion. On a general systemic level, diuresis increases in the urinary system, and the cardiovascular system, lung volumes, and endocrine functions return to their former states over time. Postpartum care includes early mobilization, appropriate diet, promotion of breastfeeding, hygiene, and the follow-up of the temporary postpartum blues, which can be seen at a rate of 70%. Important complications of this period include puerperal fever, endometritis, wound dehiscence, urinary tract infections, and lactation problems such as breast engorgement, mastitis, and breast abscess.
