Özet

Travma, çok disiplinli ve profesyonel bir anestezi yönetimi gerektiren, hayati tehlike arz eden acil tıbbi bir durumdur. Travmalı hastalarda ilk yardımın önceliği, yaşamı tehdit eden unsurları ortadan kaldırmak, özellikle de hava yolu açıklığı ve perfüzyonu hızlıca sağlamaktır. Solunum yolunun tıkanması durumunda "Bak-Dinle-Hisset" yöntemiyle değerlendirme yapılmalı; dilin geriye kaçmasını önlemek için Başı geriye itme, Çeneyi öne çekme veya servikal yaralanma şüphesinde Jaw Thrust manevraları ile orofaringeal tüpler kullanılmalıdır. Travma sonrası gelişen şok ise yetersiz doku perfüzyonuyla karakterize olup çoğunlukla hemorajiktir. Şok tanısında taşikardi, takipne, hipotansiyon ve oligüri gibi klinik bulgular esastır. Akut kan kaybının derecesine göre dört sınıfa ayrılan hemorajik şok tedavisinde öncelik, hava yolunun açılması, solunumun desteklenmesi ve kanamanın durdurulmasıdır. Erken dönemde kristalloid solüsyonlar (özellikle Ringer Laktat) hızla verilerek mikrosirkülasyon desteklenmeli, ciddi kayıplarda ise kan transfüzyonuna başlanmalıdır. Tedavinin takibinde idrar atımı, klinik tablo, asit-baz dengesi ve CVP monitorizasyonu kritik rol oynar. Yetersiz yanıt alınan dirençli olgularda nonhemorajik şok nedenleri araştırılmalı veya acil cerrahi düşünülmelidir.

Trauma is a life-threatening medical emergency that disrupts the normal homeostatic mechanism and requires a multidisciplinary approach, particularly professional anesthesia management. The priority in first aid for trauma patients is to eliminate life-threatening factors, primarily by ensuring airway patency and tissue perfusion. Airway obstructions must be evaluated using the "look-listen-feel" method, and manual maneuvers such as head-tilt/chin-lift or, in cases of suspected cervical injury, the jaw-thrust maneuver should be performed alongside the use of oropharyngeal tubes to prevent the tongue from falling back. Shock developing after trauma is characterized by inadequate tissue perfusion and is most commonly hemorrhagic in nature, with diagnosis based on clinical findings such as tachycardia, tachypnea, hypotension, oliguria, and mental agitation. Hemorrhagic shock is classified into four stages based on the percentage of acute blood volume loss, and the general management sequence involves opening the airway, supporting ventilation, controlling the hemorrhage, and establishing rapid fluid resuscitation. Initially, crystalloid solutions like Ringer's lactate are infused rapidly to restore intravascular volume and microcirculation, whereas blood transfusion is initiated for severe blood loss or in patients showing transient or minimal response to initial fluid therapy. Monitoring urine output, acid-base balance, central venous pressure, and oxygen saturation is essential to evaluate the adequacy of resuscitation and to guide definitive treatment or urgent surgical interventions.
