Özet

Bu metin, özofagus (yemek borusu), mide, duodenum (oniki parmak bağırsağı) ve ince bağırsak yaralanmalarını, yabancı cisim yutulmasını ve kostik madde hasarlarını anatomik, klinik, tanısal ve tedavi odaklı ele almaktadır. Yaklaşık 25 cm uzunluğundaki özofagusun üç anatomik darlığı, yabancı cisimlerin ve tıkanıklıkların en sık görüldüğü yerlerdir. Kostik yaralanmalarda asitler koagülasyon nekrozuna, alkaliler ise daha derin hasar veren likefaksiyon nekrozuna yol açar. Akut dönem şikayetleri yerini kronik dönemde sıklıkla (%20-25) darlık (sitriktür) gelişimine bırakır ve bu durum dilatasyon veya cerrahi müdahale (özofajektomi, kolon transpozisyonu) gerektirebilir. Mide, duodenum ve ince bağırsak yaralanmaları ise künt/penetran travmalar, yabancı cisimler veya iyatrojenik nedenlerle oluşabilir. Tanıda direkt grafi, kontrastlı pasaj grafileri ve bilgisayarlı tomografi ile mediyastinal/batın içi serbest havanın veya kontrast ekstravazasyonunun gösterilmesi kritiktir. Duodenum yaralanmalarında retroperitoneal yerleşim nedeniyle semptomlar 24-36 saat gecikebilir; küçük perforasyonlar primer onarılırken, geniş defektlerde Roux-en-Y gibi cerrahi yöntemler kullanılır. Jejunum ve ileum perforasyonlarında da altın standart kontrastlı bilgisayarlı tomografidir. Duvar bütünlüğünün tam kat bozulduğu perforasyonlarda ve iyileşmeyen obstrüksiyonlarda acil cerrahi eksplorasyon, debridman, primer onarım veya segmental rezeksiyon ve anastomoz hayati önem taşır.

This text examines injuries, foreign body ingestions, and caustic damages of the esophagus, stomach, duodenum, and small intestines from anatomical, clinical, diagnostic, and therapeutic perspectives. The esophagus, approximately 25 cm long, has three anatomical narrowings where foreign bodies and obstructions most frequently occur. In caustic injuries, acids cause coagulation necrosis, while alkalis lead to liquefaction necrosis, causing deeper damage. Acute symptoms often transition into stricture development (%20-25) in the chronic phase, which may require dilatation or surgical intervention such as esophagectomy or colon transposition. Stomach, duodenum, and small intestine injuries can result from blunt/penetrating traumas, foreign bodies, or iatrogenic causes. In diagnosis, demonstrating free mediastinal/intra-abdominal air or contrast extravasation via direct radiography, contrast passage radiographs, and computed tomography is critical. Due to the retroperitoneal location of the duodenum, symptoms of duodenal injuries may be delayed by 24-36 hours; while small perforations are repaired primarily, large defects require surgical methods like Roux-en-Y. Contrast-enhanced computed tomography is the gold standard for jejunum and ileum perforations. In full-thickness perforations disrupting wall integrity and unresolving obstructions, urgent surgical exploration, debridement, primary repair, or segmental resection and anastomosis are vital.
