Travma Sonrası Komplikasyonlar ve Tedavisi
Özet
Karın travmaları, tüm travma hastalarının yaklaşık yüzde 15'ini etkileyen ve zamanında tanı ile uygun cerrahi müdahale gerektiren hayati durumlardır. Yaralanmalar künt (düşme, kaza) veya penetran (bıçaklanma, ateşli silah) mekanizmalarla oluşur; künt yaralanmaların tespiti klinik belirtilerin başlangıçta gizli kalabilmesi nedeniyle daha zordur. Tanı sürecinde ultrason (FAST), bilgisayarlı tomografi (BT) ve tanısal laparoskopi gibi yöntemler hastanın hemodinamik stabilitesine göre seçilir. Stabil olmayan hastalarda acil travma laparotomisi ile kanama ve kontaminasyon kontrolü sağlanırken; asidoz, hipotermi ve koagülopatiden oluşan ölümcül üçlü riski altındaki hastalarda hasar kontrol cerrahisi uygulanarak kesin tedavi ikinci bakış ameliyatına bırakılır. Diyafram, mide, duodenum, pankreas, barsaklar, böbrek, mesane, karaciğer ve dalak gibi spesifik organ yaralanmalarında primer onarım, rezeksiyon veya konservatif yaklaşımlar tercih edilir. Tedavi edilmeyen doku ödemi veya sepsis, intraabdominal basıncı artırarak abdominal kompartman sendromuna yol açabilir. Yüksek şüphe indeksi, mekanizmaya uygun objektif değerlendirme ve ölümcül üçlüye dikkat edilmesi mortaliteyi azaltan en kritik faktörlerdir.
Abdominal traumas are critical conditions affecting approximately 15 percent of all trauma patients, requiring timely diagnosis and appropriate surgical intervention. Injuries occur through blunt (falls, accidents) or penetrating (stabbings, firearms) mechanisms, with blunt injuries being harder to detect due to potentially hidden initial clinical signs. In the diagnostic process, methods such as ultrasound (FAST), computed tomography (CT), and diagnostic laparoscopy are selected based on the patient's hemodynamic stability. While emergency trauma laparotomy ensures bleeding and contamination control in unstable patients, damage control surgery is applied for patients at risk of the lethal triad consisting of acidosis, hypothermia, and coagulopathy, leaving definitive treatment to a second-look operation. Primary repair, resection, or conservative approaches are preferred in specific organ injuries such as the diaphragm, stomach, duodenum, pancreas, intestines, kidneys, bladder, liver, and spleen. Untreated tissue edema or sepsis can increase intra-abdominal pressure, leading to abdominal compartment syndrome. A high index of suspicion, objective evaluation appropriate to the injury mechanism, and awareness of the lethal triad are the most critical factors in reducing mortality.
