Ekzokrin Pankreas Hastalıkları
Özet
Ekzokrin pankreas yetmezliği (EPY), pankreastan duodenuma yetersiz enzim iletilmesiyle oluşan ve besinlerin, özellikle de yağların emilim bozukluğuna yol açan bir hastalıktır. Akut ve kronik pankreatit, cerrahi rezeksiyonlar, kistik fibrozis ve diyabet gibi durumlar bu hastalığın etiyolojisinde yer alır. Tanısı, spesifik semptomların olmaması ve testlerin uygulama zorlukları nedeniyle güçtür; ancak dışkıda elastaz-1 düzeyi ölçümü tanı için klinik pratikte sıkça tercih edilen, spesifik bir yöntemdir. EPY klinikte steatore, kilo kaybı, karın ağrısı, şişkinlik ve yağda eriyen vitaminlerin eksikliğine bağlı metabolik kemik hastalıkları ile kendini gösterir. Temel tedavisi pankreatik enzim replasman tedavisidir (PERT). PERT, yağ ve nitrojen emilimini artırarak hastaların yaşam kalitesini belirgin şekilde iyileştirir; ancak hastaların domuz kaynaklı ilaçlara karşı isteksizliği ve yanlış doz kullanımı tedavi başarısını düşürebilir.
Exocrine pancreatic insufficiency (EPI) is a clinical condition characterized by the inadequate delivery of pancreatic enzymes to the duodenum, leading to malabsorption of nutrients, particularly fats. The etiology includes acute and chronic pancreatitis, pancreatic resections, cystic fibrosis, and diabetes. Diagnosis remains challenging due to non-specific symptoms and the complexity of direct diagnostic tests; however, fecal elastase-1 measurement serves as a highly specific and practical tool in current clinical management. Clinically, EPI presents with steatorrhea, weight loss, abdominal pain, bloating, and metabolic bone diseases secondary to fat-soluble vitamin deficiencies. The cornerstone of management is pancreatic enzyme replacement therapy (PERT). PERT significantly improves fat and nitrogen absorption, thereby enhancing the patient's overall quality of life. Nonetheless, treatment compliance issues, such as reluctance toward porcine-derived enzymes and improper dosing intervals, often diminish therapeutic success.
