Uyku İle Genel İlgili Tanımlar ve Yeni Uyku Bozukluklarının Sınıflaması

Özet

Sağlıklı bir uyku, bireyin yaşamının yaklaşık üçte birini kapsayan, NonREM ve REM evrelerinden oluşan, organizmanın yenilenmesi, enerji dengesinin korunması ve zihinsel performans için kritik öneme sahip biyolojik bir süreçtir. International Classification of Sleep Disorders (ICSD-3) standartlarına göre uyku bozuklukları yedi ana başlık altında sınıflandırılmakta; insomnialar, uykuda solunum bozuklukları ve uyku ile ilişkili hipoventilasyon sendromları bu kapsamda öne çıkmaktadır. İnsomnia, haftada en az üç gün tekrarlayan ve süresine göre kısa süreli ya da kronik olarak ayrılan uykusuzluk durumunu ifade eder. Uykuda solunum bozuklukları ise apne, hipopne, arousal ve RERA gibi parametrelerle tanımlanarak obstrüktif, santral ve mikst tip apneler ile obezite hipoventilasyon sendromu gibi hayati risk taşıyan klinik tabloları içermektedir. Güncel kılavuzlar, Polisomnografi (PSG) ve sınırlı parametreli cihazların (OCST) tanısal kullanım standartlarını belirleyerek klinisyenlere etyoloji, patogenez ve tedavi süreçlerinde ortak bir dünya standardı sunmaktadır.

Healthy sleep is a critical biological process occupying approximately one-third of an individual's life, consisting of NonREM and REM stages essential for organismal rejuvenation, metabolic energy restoration, and cognitive performance. Under the International Classification of Sleep Disorders (ICSD-3) standards, sleep disorders are categorized into seven main groups, highlighting insomnias, sleep-related breathing disorders, and sleep-related hypoventilation syndromes. Insomnia represents a sleeplessness condition occurring at least three days a week, classified as short-term or chronic based on duration. Sleep-related breathing disorders are defined by parameters such as apnea, hypopnea, arousal, and RERA, encompassing clinically high-risk conditions including obstructive, central, and mixed apneas, alongside obesity hypoventilation syndrome. Recent guidelines establish diagnostic criteria for Polysomnography (PSG) and Out of Center Sleep Testing (OCST), offering clinicians a global consensus to standardize etiology, pathogenesis, and target therapeutic approaches.
