Özet

Polisomnografi (PSG) eşliğinde gerçekleştirilen çoklu uyku latansı testi (MSLT), uyanıklığı sürdürme testi (MWT), periyodik ekstremite hareketleri (PLM) ve nokturnal penil tümesans (NPT) testleri, uyku bozukluklarının tanı ve takibinde kritik roller üstlenmektedir. Gündüz uykululuk eğilimini nesnel olarak ölçen MSLT; narkolepsi ve idiyopatik hipersomni tanısında belirleyici bir kriter olup, Amerikan Uyku Tıbbı Akademisi (AASM) protokollerine göre iki hafta öncesinden ilaçların kesilmesini ve test öncesi gece en az 6 saatlik bir NPSG kaydını zorunlu kılar. MSLT'nin bir modifikasyonu olan MWT ise bireyin loş ve sessiz bir ortamda uyanık kalabilme yeteneğini ölçerek, özellikle uygulanan tedavilerin etkinliğini değerlendirmek amacıyla tercih edilir. Uykuya bağlı hareket bozukluklarının tespitinde kullanılan PLM skorlaması, bacaklardaki tekrarlayan stereotipik anterior tibialis kası hareketlerini esas alır; saatte 15'ten fazla tekrar durumu periyodik ekstremite hareket bozukluğu (PLMD) tanısına işaret edebilir. Son olarak NPT, penis tabanı ve glansına yerleştirilen halka elektrotlar aracılığıyla fazik REM uykusundaki ereksiyon döngülerini kaydederek, organik ve psikojenik impotansın ayırıcı tanısında sınırlı bir klinik fayda sağlar.

Polysomnography (PSG) accompanied by the multiple sleep latency test (MSLT), maintenance of wakefulness test (MWT), periodic limb movements (PLM), and nocturnal penile tumescence (NPT) plays a crucial role in the diagnosis and monitoring of various sleep disorders. The MSLT, which objectively measures daytime sleepiness, is a decisive criterion for diagnosing narcolepsy and idiopathic hypersomnia; it requires discontinuing REM-suppressing medications two weeks prior and documenting at least 6 hours of sleep during a preceding NPSG night based on AASM protocols. Serving as a modification of the MSLT, the MWT measures an individual's ability to remain awake in a dim and quiet environment, making it highly valuable for evaluating treatment outcomes. For sleep-related movement disorders, PLM scoring relies on repetitive, stereotypic movements of the anterior tibialis muscle, where an index greater than 15 movements per hour can confirm periodic limb movement disorder (PLMD). Lastly, NPT monitors erection cycles during phasic REM sleep via ring electrodes placed around the penis, offering limited clinical utility in the differential diagnosis of organic and psychogenic impotence.
