Polisomnografi Dışı Yardımcı Tanı Yöntemleri-Anketler
Özet
Obstrüktif Uyku Apne Sendromu (OSAS) tanısında altın standart yöntem polisomnografi (PSG) olsa da yüksek maliyet ve lojistik nedenlerden dolayı herkese sunulması zordur. Bu nedenle risk altındaki bireyleri tespit etmek, PSG için doğru hastaları seçmek ve bilimsel çalışmalarda ortak dil birliği sağlamak amacıyla polisomnografi dışı yardımcı tanı yöntemleri ve anketler geliştirilmiştir. Bu kapsamda kullanılan Epworth Uykululuk Ölçeği, 8 sorudan oluşan ve 10 puan üzerinde gündüz aşırı uyku halini işaret eden, Türkçe validasyonu yapılmış bir testtir. Stanford Uykululuk Ölçeği gündüz uyku halini 1 ile 7 arasında derecelendirirken, Pitsburg Uyku Kalitesi Ölçeği 7 ana başlıkta son 1 aydaki uyku kalitesini değerlendirir ve 5 puan üzeri kötü uyku kalitesini gösterir. Toplum taramalarında kullanılan Berlin Uyku Anketi ise 3 kategoride 10 sorudan oluşur ve en az 2 kategorinin pozitif olması durumunda hastayı yüksek riskli kabul eder. Son olarak, anestezistler tarafından preoperatif değerlendirmede sıklıkla tercih edilen STOP anketi 4 sorudan oluşurken, buna beden kitle indeksi, yaş, boyun çevresi ve cinsiyet kriterlerinin eklenmesiyle oluşturulan STOP-BANG anketi 8 soru üzerinden OSAS riskini belirlemede kullanılır.
Although polysomnography (PSG) is the gold standard for diagnosing Obstructive Sleep Apnea Syndrome (OSAS), providing it to everyone is challenging due to high costs and logistical limitations. Consequently, non-polysomnographic screening tools and questionnaires have been developed to identify high-risk individuals, optimize patient selection for PSG, and establish a common language in clinical research. Among these tools, the Epworth Sleepiness Scale is an 8-item questionnaire validated in Turkish, where a score of 10 or above indicates excessive daytime sleepiness. The Stanford Sleepiness Scale subjectively rates daytime sleepiness on a scale from 1 to 7, while the Pittsburgh Sleep Quality Index evaluates sleep quality over the past month across 7 components, with a total score of 5 or more indicating poor sleep quality. For community screenings, the Berlin Sleep Questionnaire consists of 10 questions in 3 categories, classifying individuals as high-risk if 2 or more categories are positive. Finally, the 4-item STOP questionnaire and its expanded 8-item version, the STOP-BANG questionnaire—which incorporates body mass index, age, neck circumference, and gender—are widely used by anesthesiologists for preoperative OSAS risk assessment.
