Obstrüktif Uyku Apnesi Sendromu ve Kardiyovasküler Sonuçları
Özet
Obstrüktif uyku apnesi sendromu (OUAS), uyku sırasında üst hava yolunun tıkanmasıyla karakterize, önemli morbidite ve mortaliteye yol açan ciddi bir solunum bozukluğudur. OUAS esnasında meydana gelen aralıklı hipoksemi, hiperkapni, otonomik sistem değişiklikleri ve sempatik aktivite artışı gibi patofizyolojik mekanizmalar; endotel disfonksiyonu, oksidatif stres ve inflamasyonu tetiklemektedir. Bu süreçler; sistemik hipertansiyon, koroner arter hastalığı, sol kalp yetmezliği, pulmoner hipertansiyon, kardiyak aritmiler, serebrovasküler hastalıklar ve uykuda ani ölüm gibi çok ciddi kardiyovasküler sonuçlara zemin hazırlamaktadır. Araştırmalar, özellikle dirençli hipertansiyon ve miyokard infarktüsü geçiren hastalarda OUAS prevalansının son derece yüksek olduğunu ortaya koymaktadır. Erken tanı konulması ve sürekli pozitif hava yolu basıncı (CPAP) gibi etkin tıbbi tedavilerin uygulanması, uykudaki solunum problemlerini düzeltmenin yanı sıra kardiyovasküler komplikasyonların gerilemesinde, sol ventrikül hipertrofisinin azalmasında ve uzun vadeli mortalite riskinin düşürülmesinde hayati bir rol oynamaktadır.
Obstructive sleep apnea syndrome (OSAS) is a severe respiratory disorder characterized by upper airway obstruction during sleep, leading to significant morbidity and mortality. Pathophysiological mechanisms during OSAS, such as intermittent hypoxemia, hypercapnia, autonomic alterations, and increased sympathetic activity, trigger endothelial dysfunction, oxidative stress, and inflammation. These processes predispose patients to serious cardiovascular consequences, including systemic hypertension, coronary artery disease, left heart failure, pulmonary hypertension, cardiac arrhythmias, cerebrovascular disease, and sudden cardiac death during sleep. Research highlights an extremely high prevalence of OSAS, particularly among patients with resistant hypertension and myocardial infarction. Early diagnosis and the implementation of effective medical treatments, such as continuous positive airway pressure (CPAP), play a vital role not only in correcting sleep-related breathing sleep disturbances but also in reversing cardiovascular complications, reducing left ventricular hypertrophy, and lowering long-term mortality risks.
