PAP Tedavisi Endikasyonları ve Cihaz Seçimi
Özet
Pozitif havayolu basınç (PAP) tedavisi, obstruktif uyku apne sendromu (OUAS) ve diğer solunumsal uyku bozukluklarının tedavisinde 1981 yılından beri kullanılan temel yöntemdir. American Academy of Sleep Medicine (AASM) kılavuzlarına göre tedavi endikasyonları, Apne Hipopne İndeksi (AHI) değerinin 15’in üzerinde olması ya da AHI değerinin 5-15 arasında olup aşırı uykuluk hali ve kardiyovasküler ek hastalıklar gibi semptomların eşlik etmesi durumudur. Tedavinin temel amacı, üst solunum yolunun uyku sırasında açık kalmasını sağlayarak apne, hipopne, oksijen desatürasyonu ve uyanmaları (arousal) engellemektir. Tedavide hastanın klinik durumuna göre CPAP, otomatik CPAP (APAP), iki seviyeli sabit basınç sağlayan BPAP, solunum sayısı ayarlı BPAP-ST, gelişmiş bir mod olan AVAPS ve Cheyne-Stokes solunumu gibi özel durumlarda tercih edilen ASV cihazları kullanılmaktadır. Cihazların gecede 4 saatten fazla ve toplam uyku süresinin %70’inden fazlasında kullanılması etkin tedavi için şarttır. Cihaz seçimi kadar, hastanın anatomik yapısına uygun nazal, oronazal, nazal yastıkçık veya oral maske seçimi ile ısıtıcı/nemlendirici gibi aksesuarların doğru kullanımı da hasta uyumunu ve tedavi başarısını doğrudan etkileyen kritik unsurlardır.
Positive airway pressure (PAP) therapy has been the primary method in treating obstructive sleep apnea syndrome (OSAS) and respiratory sleep disorders since 1981. According to American Academy of Sleep Medicine (AASM) guidelines, the indications for treatment are defined as an Apnea-Hypopnea Index (AHI) greater than 15, or an AHI between 5-15 accompanied by symptoms such as excessive sleepiness and cardiovascular comorbidities. The main objective of the therapy is to keep the upper airway open during sleep, thereby preventing apnea, hypopnea, oxygen desaturation, and arousals. Depending on the patient's clinical condition, devices such as CPAP, automatic CPAP (APAP), bilevel PAP (BPAP), time-modulated BPAP-ST, AVAPS, and adaptive servo-ventilation (ASV) for Cheyne-Stokes respiration are utilized. Effective therapy requires the device to be used for more than 4 hours per night and over 70% of the total sleep duration. Along with device selection, choosing the appropriate nasal, oronasal, nasal pillow, or oral mask tailored to the patient's anatomy, as well as the correct use of accessories like humidifiers, are critical factors that directly influence patient compliance and treatment success.
